Background:Hyperglycemia was one of the main reasons induced endothelial cell damage and dysfunction.Inflammation theory has been widely recognized in the pathogenesis of type 2 diabetes.Oxidative stress also an important factor in diabetic vascular endothelial injury.Tumor necrosis factor-α(TNF-α),Interleukin-6(IL-6),Reactive oxygen species(ROS) were important roles in the development of diabetic vascular complications.Objective:Through clinical and experimental research,exploration of removing phlegm and blood-stasis together to prevent and control high glucose-induced vascular endothelial injury, in order to provide clinical and experimental bases of preventing and controlling diabetic vascular lesions.Materials and Methods:1.Clinical data and methods:①.According to type 2 diabetes and TCM phlegm-blood stasis diagnostic criteria,150 cases were selected,including 78 cases of men and women in 72 patients,with an average age 59.87±10.87 years old.②.Patients with Xionglou Tongmai decoction of removing phlegm and blood-stasis together were classified as the treatment group,not taking medicine Xiongloutongmai Prescription of removing phlegm and blood-stasis together classified as the control group.③.TNF-α,IL-6 were measured by the TNF-α,IL-6 test kit provided by Jingmei Bio-engineering Co,Ltd in Shenzhen.2.Experimental materials and methods:①.Cells and culture conditions:Human umbilical vein endothelial cells(ECV-304),provided by China Center for Type Culture Collection(CCTCC),cultured in temperature 37℃,5% CO2 and 95%humidity in the sterile incubator.②.M199 medium,bovine serum,trypsin etc.the United States,PIERCE HyClone products.③.Cells MTT experiment was measured by ELx800 Universal Microplate Reader.Reactive oxygen species(ROS) were measured by the kit provided by Jiemei Gene Pharmaceutical Technology Co,Ltd in Shanghai.Results:①.There was no significantly difference about the serum levels of TNF-αbetween the treatment group and the control group before treatment(P>0.05).The serum levels of TNF-αhad no significantly difference between two groups after treatment(P>0.05).But the difference was significant about the variety of TNF-a levels after treatment between the treatment group and the control group(P<0.05).②.In the MTT experiment,the statistical analysis of the OD value in the human vascular endothelial cells of the groups:There was no significant difference(P>0.05),the standard 1/600 drug group and the standard 1/300 drug group compared with the standard group.There were very significant differences(P<0.01),the Standard 1/150 group and the standard 1/100 group compared with the standard group.There was no significant difference(P>0.05) between the Standard 1/100 group and the blank control group.③.The statistical analysis of the reactive oxygen species(ROS) value in the human vascular endothelial cells of the groups:The difference was significant(P<0.05) between the Standard 1/300 drug group and the standard group.The difference were very significant(P<0.01) between the high glucose group and the standard group.The difference were very significant (P<0.01) between the 1/600 drug-glucose group and the 1/300 drug-glucose group.The 1/600 drug-glucose group and the 1/300 drug-glucose group showed no significant difference(P>0.05).The difference was very significant between the 1/150 drug-glucose group and the high glucose group(P<0.01).④.Before treatment,there was no significantly difference about the serum levels of fasting blood glucose(FBG) between the treatment group and the control group(P>0.05).After treatment,the serum levels of FBG decreased in both groups(the treatment group:P<0.01,the control group:P<0.05),but the serum levels of FBG had no significantly difference between the two groups after treatment(P>0.05).The variety of FBG had no significant difference between groups(P>0.05).⑤.The clinical TCM symptom score dropped in both groups(P<0.01),and it was lower in the treatment group than that in the control group(P<0.05).There were significant differences(P<0.05) in clinical Syndrome efficacy between the two groups(P<0.05).⑥.There was no significantly difference about the serum levels of IL-6 between the treatment group and the control group before treatment(P>0.05).The serum levels of IL-6 had no significantly difference between the two groups after treatment(P>0.05).And the difference was no significant about the variety of IL-6 levels after treatment between the two groups(P<0.05).Conclusion:Through removing phlegm and blood-stasis together,Xionglou Tongmai decoction could improve the clinical syndromes and reduce the levels of TNF-a in the patients with type 2 diabetes.Xionglou Tongmai decoction could significantly reduce high levels of reactive oxygen species in the cells caused by the high concentration of glucose,also reduce reactive oxygen species level of the cultured vascular cells in the general concentration of glucose. Xionglou Tongmai decoction had no significant impact to fasting blood glucose.It may play its anti-inflammatory and anti-oxidative stress through non lower blood glucose channels. Xionglou Tongmai decoction is beneficial to reduce diabetic vascular endothelial injury and delay the process of diabetic vascular endothelial dysfunction.Furthermor,it may play the double roles of preventing and treating diabetic vascular endothelial injury.
