Objective:To observe effects of Liuweidihuang capsula mollis and Ginkgo biloca leave prearation on carotid artery intima-media thickness and inflammatory factors in patients with type 2 diabetes mellitus and perform economic health evaluation.Methods:A randomized,double-blind,positive drug controlled trial was conducted. One group was given the west medicine simplely and the other group was given the Chinese medicine and west medicine.There was 108 patients.They were divided into two groups randomizely.54 patients were taken into the tcm group and the other 54 patients were into the comparison group.The tcm group was treated with Liuweidihuang capsula mollis and Ginkgo biloca leave prearation.The comparison group was treated with placebo. All patients were treated for 18 months.103 patients completed the experiment at 6th month,and 87 patients completed the experiment at 18th month.Finally,21 patients exfoliated.This apper choosed 62 patients with the carotid artery intima-media thickness increased,and economic health evaluation was performed by using cost effectiveness analysis.The observed curative effect indexes include ante cibos blood glucose, poswasibos blood glucose,blood pressure,HbA1c,serum lipid,tumor necrosis factor -α,interleukin-6 and carotid artery intima-media thickness.Results:①Treats for 18 months,the tcm group and comparison group HbA1c basically achieved the experimental control request,no significant difference was found between the two groups(p>0.05).②Treats for 18 months to tcm group TC,LDL-C and the HDL-C improvement has surpasses the comparison group,two groups have no significant difference(p>0.05).③Treats for 18 months to tcm group the IL-6,TNF-αlevel are lower than the comparison group,no significant difference was found between the two groups(p>0.05).④Treats for 18months to tcm group IMT to increase the degree to be smaller than the comparison group,two groups have no significant difference(p>0.05); take IMT increased more than 0.2mm as the remarkable accumulation,tcm group IMT no remarkable accumulation rate is bigger than the comparison group,the difference was significant(p<0.05);The tcm group comparatively compares the group relative risk to drop 29.9%.⑤Treats for 18 months the data statistics to demonstrate IMT and IL-6 have a stronger correlation(r=0.265,p<0.05);IMT and TNF-αdo not have the correlation(p>0.05).⑥Cost-effectiveness analysis suggest that,the tcm group IMT no remarkable accumulation rate increase 1%need 187 yuan per-capita,the comparison group need 264 yuan per-capita.The cost reduces 29.9%between two group.Conclusion:First,experimental group TC,LDL-C and the HDL-C improvement has surpasses the comparison group,Suggested that Liuweidihuang capsula mollis and Ginkgo biloca leave prearation have a certain improving effect on blood lipid.Second, take IMT increased more than 0.2mm as the remarkable accumulation,experimental group IMT remarkable accumulation delaying rate is bigger than the comparison group, Suggested that using the traditional chinese medicine treatment in the early time to be allowed to delay the carotid artery intima-media thickness accumulation,reduces the happening of vascular complication.Third,treats for 18 months the data statistics to demonstrate IMT and IL-6 have a stronger correlation,Suggested that we can achieve our goal to delay IMT thicken through relieving inflammation and decreasing the release of inflammation factors.The last,the cost- effectiveness is lower when combined by tradition chinese medicine and western medicine on delay macro vascular disease,Suggested that traditional chinese medicine is economy.
