Association Of Serum Transforming Growth Factor-(TGF-β1), Laminin And Type Ⅳ Collagen With Diabetic Nephropathy In Type2 Diabetic Patients

OBJECTIVE:To investigate the relationship between serum level of typeⅣcollagen,Laminin,TGF-β1 and diabetic nephropathy in type2 diabetic patients.Moreover, Investigate the Significance in early diagnosis of diabetic nephropathy.METHODS:Serum levels of typeⅣcollagen,laminin(LAN),and TGF-β1 were determined with ELISA method in 61 patients with type 2 diabetes mellitus and 18 normal controls.Diabetic patients were divided into patients with diabetic nephropathy (DN) and patients without DN(DM,UAER＜30mg/24h) according to the urine albumin elution rate(UAER).Moreover,patients with DN were divided into patients w -ith micro album in urine(MA,(UAER 30～300mg/24h) and patients with- clinical DN (CDN,UAER＞300mg24h).The data were analysised by SPSS(Statistical Program for Social Sciences) 12.0 software.Variances between groups were compared with One-w-ay ANOVA analysis procedure.Simple factor was analysised by linear correlation procedures. Multiple factors were analysised by multiple stepwise regression proceduers The value of serum typeⅣcollagen,LAN,and TGF-β1 in early diagnosis of DN was justed by AUROC(Area Under Receiver Operating Characteristic).RESULTS:In both patients with DN and patients without DN,serum level of typeⅣcollagen,LAN were higher than those in normal subject(P＜0.01).The differrence of serum level of typeⅣcollagen and LAN between diabetic patients is signifcant. (P＜0.01) Moreover,serum level of typeⅣcollagen and LAN between diabetic patients increase according to UAER.There were no differences when Serum TGF-β1 was compared between diabetic patients.(P＞0.05) The relationship between typeⅣcollagen, TGF-β1 and LAN were significant.Liner regression analysis in diabetic patients showed that serum typeⅣcollagen and TGF-β1 was positively correlated with fasting plasma glucose(FPG),and UAER respectively.Serum typeⅣcollagen is a more sensitive indictor in early diagnosis than LAN.CONCLUSION:Serum typeⅣcollagen,TGF-β1 and LAN were increased in patients with type2 diabetes as compared with normal subjects,and had the close relationship with the happiness and development of diabetic nephropathy(DN). Serum typeⅣcollagen can be used,as index of early DN.Serum typeⅣcollagen appears to be the predictor of severity in DN.
