Harmonious doctor-patient relationship is the basis of high-quality medical services, and good communication is an effective way to establish a harmonious relationship between doctors and patients. An effective physician-patient communication can strengthen the trust between doctors and patients, made closely doctor-patient relationship and reduce patient's contradictions. Doctor-patient communication is also an art and a skill. It is a required course and essential skill for all medical staffs and an effective way to solve the disputes between patients and doctors. In this study, we investigated and analyzed the status quo of current doctor-patient communication and found the existed problems out in a general hospital through literature review, expert advice, field research, and statistical analysis and so on. Then the corresponding improvement measures and proposals were put forward. Our research consists of five parts:Part I: Summary. Mainly focus on the research background, purpose and the significance of this study.Part II: Survey the recent developments of the doctor-patient communication research. Mainly focus on the relevant definitions and concepts of doctor-patient communication and current research background in China and abroad. At last, summarize the problems of the current domestic study.Part III: Theoretical discussion of doctor-patient communication. Mainly focus on the status quo and the causes of doctor-patient relationship in China. Introduce the theoretical basis of doctor-patient communication through analyzing patient's mental, health law and medical model in details. Indicate the importance of doctor-patient communication and the impact factors of misunderstanding between them at the same time.Part IV: Investigation and analysis of the status quo of the doctor-patient communication in a general hospital. It is the core part of this thesis. Firstly, we designed a draft of the questionnaire in accordance with the literatures about doctor-patient communication. Then, after discussing with experts, doing some pre-test and analyzing the reliability and validity, we finally made the draft into the formal questionnaire. And the formal questionnaire has two forms, one is for patients and the other one is for medical staffs. Each form of the questionnaire is designed in accordance with four different levels and the contents of the questionnaires are comprehensive and rational. Then using Chester sampling method, we investigated 260 patients and 267 medical staffs at the general hospital with the questionnaires. Through data analysis we found: The patients are all agree with that the high confidence in the medical staffs and the good doctor-patient communication between the two sides may play an important role in the course of diagnosis and treatment but it still need to be improved; we also discovered that there are six factors can constrain the improvement of doctor-patient communication from the data including different demand of communication between doctors and patients; medical staffs lack of humane care spirit and communication skills; neglect of patients'informed consent right, irrational allocation of resources, patients lack of medical knowledge.Part V: Recommendations and countermeasures to improve the doctor-patient communication. In view of the problems and the status quo of the doctor-patient relationship, the recommendations and countermeasures were put forwarded from four aspects, including hospital management, government, patients and public media. Among the four aspects, hospital management is the most important one which play the lead role in the improvement of the doctor-patient communication. In the strengthening of hospital management, we put forward nine recommendations and countermeasures, include establishing a"Patient-centered"service philosophy establishment, constructing a effective long-term mechanism to enhance the doctor-patient communication, improving the health-care quality, allocating human resources rationally, strengthening the medical-expenditure management, respecting for patients'informed consent right fully, increasing communication-skill training to medical staffs, intensifying humanities education to medical students and health education to patients. The innovation of this research is to investigate the problems of doctor-patient communication statistically by using a comprehensive and scientific questionnaire. It also lists some recommendations and countermeasures to improve the status quo of current doctor-patient relationship. It has a practice instruction function in the doctor-patient relationship improvement and a wonderful guiding sense in the case study at the same time.
