Hepatitis B virus(HBV) infection is a major global health concern and is the most common cause of chronic liver disease including Acute and chronic hepatitis B,liver cirrhosis,liver cancer worldwide.There are 350,000,000 asymptom HBV carriers in the world.At present,based on 8%nucleartides heterogeneous in whole genome or 4% nucleartides heterogeneous in S gene sequence,8 kind of genotypies are divided including HBV A-H genotype.It's different on the feature,generation,development,transform and relationship between the liver disease caused by those 8 kinds of genotypes.The main target of the study is investigating the relationship between HBV genotypes and spectrum of disease. We also inquire into the different anti-virus effect of interferon-αon each HBV genotypes.This research will bring an important guiding effect in adminstrating proper medicine in clinical.A total of 210 patients infected by HBV were subjects.HBV infection in sera was positive by fluorescent quantitation PCR.There are 52 asymptom HBV carriers,112 chronic hepatitis B infectioners and 46 hepatitis B liver cirrhosis patients.We test the genotype by direct-gene sequencing with serum specimen,and compare the diference between sequenced genes and known HBV genotypes submitted in Genbank.Finally, the genotype was comfirmed.Then we choose 60 chronic hepatitis B infected patients including 30 of genotype B infected and 30 of genotype C infected.Those patients would be treated by interferon therapy for 1 year.We have monitored the biochemical,serological and virological response regularly.Then,χ2- test was made between each genotype group to determine the different anti-virus effect.Genotype distribution in asymptom HBV carrier group:genotype B 55.8%(29/52), genotype C 44.2%(23/52);Genotype distribution in chronic hepatitis B group:genotype B 25%(28/132),genotype C 75%(84/132);Genotype distribution in hepatitis B liver cirrhosis group:genotype B 13%(6/46),genotype C 87%(40/46);There is not found genotype A,D,E,F,G and H in Shenyang territory.Genotype distribution comparison between asymptom HBV carrier group and chronic hepatitis B group _χ2=14.827 P=0.000;Genotype distribution comparison between asymptom HBV carrier group and hepatitis B liver cirrhosis group _χ2=19.407 P=0.000;Genotype distribution comparison between chronic hepatitis B group and hepatitis B liver cirrhosis group _χ2=2.760 P=0.097.Therapeutic effect appraisal of interferon:HBVDNA inhibition HBVDNA darkening rate:genotype B>genotype C,it is approximately predominance difference between genotype B and genotype C(_χ2=4.344,P=0.037).HBeAg clearance and anti-HBe generate HBeAg clearance rate:genotype B>genotype C,it is approximately predominance difference between genotype B and genotype C (_χ2=5.554,P=0.018 ).ALT normal:it is no difference between genotype B and genotype C(_χ2=2.7,P=0.1).Combinated therapeutic effect(HBVDNA darkening,HBeAg clearance,ALT normal):genotype B is good than genotype C,it is approximately predominance difference between genotype B and genotype C(_χ2=5.711,P=0.017).In a word,HBV B genotype and C genotype were the dominant HBV genotypes in Shenyang city.The majority of asymptomatic HBV carriers were infected with HBV B genotype.However,The majority of chronic hepatitis patients and liver cirrhosis patients were infected with HBV C genotype.After interferon therapy,we find that different effects of interferon on the two genotypes of HBV infection.The disease recovery was evaled by HBV DNA suppression,HBeAg clearance and anti-HBe production.HBV B genotype infected patients were better than HBV C genotype infected patients.
