Abstract/Summary:
Background:The treatment of solid tumors through the inhibition of angiogenesis has achieved remarkable results. The research about the relationship between angiogenesis and hematopathy has made great progress over the last decade.Anti-angiogenesis drugs such as thalidomide has achieved good efficacy in the treatment of multiple myeloma,and this will inspire people to explore the application of anti-angiogenesis drugs in the treatment of hematopathy. Many studies indicate that angiogenesis have prognostic value in hematologic malignancies.Among the known angiogenic factors, vascular endothelial growth factor is the most important stimulator of the angiogenic process inphysiological and pathological conditions.It can promote the activation,migration, proliferation of endothelial cells and the maturation of blood vessels. There is a certain amount of VEGF in normal cycle in order to maintain normal vascular system. The studies on the expression and regulation of VEGF have shown that anemia/hypoxia can promote the expression of VEGF by many normal cells. The activation of oncogene can upregulate the expression of VEGF. The cells of hematologic malignanacies can also express VEGF and its receptors which stimulate their own secretion and proliferation of endothelial cells and promote angiogenesis in bone marrow through autocrine and paracrine. In the preliminary research, we study the MVD of bone marrow and the expression of plasma VEGF and VEGF in bone marrow of acute leukemia,lymphoma without infiltration of bone marrow and anemia, and we find that VEGF in bone marrow is more higher than the plasma VEGF in acute myeloid leukemia,and the plasma VEGF in lymphoma is higher than VEGF in bone marrow.Although they are malignant hematopathy, the expression of plasma VEGF and VEGF in bone marrow is different.This indicates the role of VEGF is different.Because there are a few cases of lymphoma,we can't analyze the significance of VEGF in bone marrow and the plasma VEGF in lymphoma.Now we increase the cases, detect the expression of plasma VEGF and VEGF in bone marrow by ELISA in malignant lymphoma and analyze the role through the combination of clinical parameters. The objective is to explore the clinical significance of VEGF in lymphoma.Methods:In this study, the plasma VEGF and VEGF in bone marrow were detected by ELISA in 26 patients with newly diagnosed malignant lymphoma (3 of them have the infiltration of bone marrow after treatment), compared with 8 healthy people.Results:1. The plasma VEGF and VEGF in bone marrow of malignant lymphoma were higher than that of healthy controls, the difference was statistically significant. The plasma VEGF was higher than VEGF in bone marrow in lymphoma, and the difference was statistically significant. There was no correlation between plasma VEGF and VEGF in bone marrow in lymphoma.2. After the infiltration of bone marrow, VEGF in bone marrow in patients with lymphoma increased, and the difference was statistically significant. The plasma VEGF also increased, but the difference was not statistically significant. 3. The expression of plasma VEGF at first in the group with infiltration of bone marrow was higher than the group without infiltration of bone marrow, but the difference was not statistically significant; The expression of VEGF in bone marrow at first in the group with infiltration of bone marrow was shorter than the group without infiltration of bone marrow, and the difference was not statistically significant.4. The plasma VEGF and VEGF in bone marrow of patients with malignant lymphoma had no correlation with gender, age and B symptoms. The patients in stageⅢorⅣhad higher value of plasma VEGF than ones in stageⅠorⅡ, and the difference was statistically significant; The patients in stageⅢorⅣhad higher value of VEGF in bone marrow than ones in stageⅠorⅡ,but there was no significant difference.5. The expression of plasma VEGF in the group with normal LDH was higher than the group with high LDH, but there was no significant difference; The expression of VEGF in bone marrow in the group with normal LDH was shorter than the group with high LDH, and there was also no significant difference. The plasma VEGF and VEGF in bone marrow in lymphoma had no correlation with LDH. The expression of plasma VEGF in the group with normalβ2-MG was higher than the group with highβ2-MG, and the difference was statistically significant; The plasma VEGF was significantly correlated withβ2-MG. The expression of VEGF in bone marrow in the group with normalβ2-MG was higher than the group with highβ2-MG, but there was also no significant difference. VEGF in bone marrow in lymphoma had no correlation withβ2-MG.Conclusions:1. There are increased plasma VEGF and VEGF in bone marrow in patients with malignant lymphoma. There are angiogenesis in malignant lymphoma.2. After the infiltration of bone marrow, VEGF in bone marrow in patients with lymphoma increases.VEGF in bone marrow may be secreted by blast cells. The dynamic changes of VEGF in bone marrow may predict to the infiltration of bone marrow to some extent.3. There are no correlation with gender, age, and B symptoms. The patients in stageⅢorⅣhave higher value of plasma VEGF than ones in stageⅠorⅡ, and the difference is statistically significant The plasma VEGF in malignant lymphoma patients are helpful in the evaluation of Dukes staging.4. The expression of plasma VEGF in the group with normalβ2-MG is higher than the group with highβ2-MG, and the difference is statistically significant; the detection of plasma VEGF has prognostic value to some certain.5. The detection of plasma VEGF is more meaningful than that of VEGF in bone marrow when the bone marrow is not infiltrated in lymphoma.
