Objective:To study the expression level and distribution of humanβ-defensin 2(hBD-2) in cholesteatoma,granulation tissue of tympanic cavity and the skin of external auditory meatus of simple type in chronic suppurative otitis media.To investigate the effect of hBD-2 in the pathogenesis and the development of the human middle ear cholesteatoma.Method:20 cholesteatoma tissue pecimens of chronic suppurative otitis media,10 granulation tissue of tympanic cavity with chronic suppurative otitis media and 10 the skin of external auditory meatus of simple type in chronic suppurative otitis media,by the hematoxylin-eosin colouring(HE) and Streptavidin-Peroxidasemethod(S-P method) of immunohistochemistry with monoclonal antibody against hBD-2,are observed in the pathological change of these three tissue and researched in the expression position of hBD-2 in these tissue and investigate quantitative change of hBD-2 in theses three tissue of three groups.The data obtained by the computer image processing system was analyzed with statistics means.Result:hBD-2 expression was faint in the skin of the external auditory meatus of simple type in chronic suppurative otitis media,strong immunostaining for hBD-2 in cholesteatoma epithelial tissues and middle ear granulation.Cholesteatoma tissue expression of hBD-2 is stronger than the granulation of chronic suppurative otitis media.The results of the immunohistochemical semi-quantitative analysis(expression of hBD-2),the external ear meatus skin of chronic suppurative otitis media(10 specimen):1 positive,weakly positive in 7 specimen,2 specimen of negative;granulation of chronic suppurative otitis media(10 specimen):1 specimen of strong positive, 7 specimen were positive,2 specimen of weakly positive;cholesteatoma epithelium(20 specimen):11 specimen of strong positive,7 specimen were positive,1 specimen weak positive,1 specimen negative.Computer image analysis the expression of hBD-2(gray value):the skin of the external auditory meatus in chronic suppurative otitis media group was 116.1089±19.4373,granulation tissue of chronic suppurative otitis media 100.8388±16.7878,cholesteatoma group was 95.7852±15.0251.There were statistically significant during these three groups.(P<0.05).Conclusion:hBD-2 in inducible expression in cholesteatom tissue and granulation tissue compared with the skin of external auditory meatus of simple type in chronic suppurative otitis media.The different expression of hBD-2 in cholesteatoma and granulation tissue may contribute to the occurrence and development of cholesteatoma.so it may be one of vital factors in the pathogenesis associated with cholesteatoma.The hBD-2 high expression in cholesteatoma tissue has played a major role in immune activity.A large number of pro-inflammatory induced expression of hBD-2,and hBD-2 attract inflammatory cells releasing pro-inflammatory,causing the bone mass destruction.The use of perioperative anti-inflammatory drugs or inhibit the expression of hBD-2 to alleviate the cholesteatoma and related epithelial hyperplasia of bone destruction.
