The Clinical Study Of Shallow Needle On Bell's Palsy In Acute Period

This article is based on the study of the Bell' s palsy.The evidence came from literature summarization and clinical research.The author have studied Bell' s palsy and made the Primary discussion and summary.Bell' s palsy is one of the most popular disease,we are easy to be suffered in spring and fall.Most of the patients find one-side of facial paralysis in the morning at first.At clinic it is shows:wrinkle on the paralyzed face disappear.The eyes can not close completely.Air can leak from the corner of the mouth.The nasal and labial groove has turned shallow and the whole mouth has slanted to one side.The patients can not lour,show teeth,plump mouth,whistle.The disease has a rapid courses,it will reach the greatest degree of facial nerve injury within hours to one or two days.This disease can ascribe to facial paralysis,koupi,distorted of mouth and eyes.The cause of the disease should be lack of qi,vain of meridian and collateral and the wind evil take advantage of the weakness of the body to invade the function of Yangming and Shaoyang meridians.It led to qi blocked and the muscle meridian lack of nutrition so they become paralyzed.Nowadays the study of the Bell' s palsy made a great progress.This article is based on the ancient and current documentary,aim to summarize the etiology and pathogenesis,acupuncture and modern treatment.The observation of the curative effect of the treatment of the shallow needle on treating the Bell' s palsy at the acute period is provided by an effective treatment of the disease.Method:in the clinical observation,choose 60 qualified patients divided into two groups randomly by the tickets the have.Treatment group use the shallow needle and western medicine during acute period,the control group only use the western medicine.Main acupoints at the first course:Yangbai,Dichang,Jiache, Sibai,Quanliao,Hegu.Differentiation syndrome acupoint:wind-heat invading meridian and collateral add Oazhui,Yifeng,Quchi:wind-cold attacking meridian and collateral add Fengchi;deficiency of qi and blood stasis add Zusanli,Sanyingjiao;blockage by wind and phlegm in the meridians add Fenglong, Neiguan.Symptom acupoints:the flat nasal and labial groove adds Yingxiang, Kouheliao;the distorted nasal and labial groove adds Shuigou,the distorted cheek groove add Chengjiang,close eye incompletely add Cuanzhu.The acupoint of the 2^th and 3^th courses on the face should be deeper than the first course. Main acupoints at the 2^th and 3^th courses:Dicang to Jiache,Jiache to Dichang, Yangbai to Yuyao,Cuanzu to Yuyao,Yingxiang to sibai.Differentiation syndrome acupoints and symptom acupoints are same to the first course.Treatment group:at the first course take all main acupoints,2 differentiation syndrome acupoints and 3 symptom acupoints each time.After regular sterilized,use the 1 inch needles to needling about 0.3～0.5 cun in the skin with quick needle insertion.Twirl the needles with twirling needling method in every 6 minutes.Then retain the needles for 30 minutes and use the TDP to radiate the paralyzed face at the same time.Use western medicine too at acute period.At the 2^th and 3^th courses the needle should be deeper.Take all main acupoints,2 differentiation syndrome acupoints and 3 symptom acupoints each time.Use the 1 inch needles to needling about 0.8cun～1 cun. When the patient gets sensation by the needle,add electrical therapy.The filiform needles will be fixed G6805-1 electrical needle machine,use sensitive and sparse wave for 30 minutes.Use the TOP to radiate the paralyzed face at he same time.control group:at acute period do not acupuncture and just use TOP and the western medicine.Use acupuncture combined with the western medicine and TDP after 8^th days.The therapy of acupuncture is same to treatment group.7 days are a period of treatment,one time treatment a day.When a period of treatment finished,take one day rest.They will take 3 periods at last.Observation indexes:the main symptoms and signs of facial paralysis.Refer to the 5th International Facial Neurosurgery Symposium recommended House-Brackmann(H-B)facial nerve grading scale to process differing levels of severity.Effect standard:the 5th International Facial Neurosurgery Symposium which recommended the House Brackmann(H-B)facial nerve gradingscale.Result:There is not significance differences between two groups in gender and age(P＞0.05):For the total scores of symptoms of both group,which represent the severity of disease,there are no obvious differences in statistics before treatment(P＞0.05).It means there are no significant differences in symptom grades between the two groups before treatment.It is comparable between the two groups.After treatment the contrast is marked or obvious marked(P＜0.01) or(P＜0.05) for treatment group in depth of forehead wrinkle,size of rima oculi,close eyes,activity of nasal vestibule,depth of nasolabial groove,aslant mouth,air leak from the cheek,taste,auditory,retroauricular pain.After treatment besides taste the contrast is marked or obvious marked (P＜0.01) or(P＜0.05) for control group in depth of forehead wrinkle,size of rima oculi,close eyes,activity of nasal vestibule,depth of nasolabial groove,aslant mouth,air leak from the cheek,auditory,retroauricular pain. It means both treatment group and control group have good effect in improving the symptoms and signs for Bell' s palsy during acute period.After treatment there are significant different between the treatment group and control group in depth of forehead wrinkle,size of rima oculi,close eyes,activity of nasal vestibule,depth of nasolabial groove,aslant mouth,air leak from the cheek, retroauricular pain(P＜0.05).It means the treatment group is superior than control group in improving the symptoms and signs for Bell' s palsy.The total effective rate of treat group is 96.7%,and the control group is 86.7%.The treatment group is superior than the control group(P＜0.05)so the curative effect of the treatment group is better than that of the control group.conclusion:Through clinical research,therapeutic effect of acupuncture of Bell' s palsy is related to point selection,needling and moxibustion methods, acupuncture treatment opportunity,and different treatment stages.The therapy of a combination of shallow needling and western medicine is significantly superior to the therapy of only western medicine during acute period.The treatment group is significantly better than the control group in improving symptoms of depth of forehead wrinkle,size of rima oculi,close eyes,activity of nasal vestibule,depth of nasolabial groove,aslant mouth, air leak from the cheek,retroauricular pain.The treatment group improve sense of taste and auditory after treatment.But the control group do not improve sense of taste.Acute stage is the best opportunity in acupuncture and moxibustion treatment for Bell palsy.
