Clinical Study Of Acute Exacerbation Of Chronic Blocking Lung Disease With Syndrome Of Phlegm And Blood Stasis Obstructing Lung Treatment With Taohechengqitang Enema

Objective:To investigate the effects of treatment with TCM drugs enema in syndrome of phlegm and blood stasis obstructing lung Chronic Blocking Lung Disease according to the theory of through exterior yang organs to treat interior yin organs and study the curative mechanism through observing proving of clinical symptom,blood routine,blood gas analysis,CRP,lung and kidney function and electrolyte,so as to find out effective measure in clinical treatment of acute exacerbation of chronic obstructive pulmonary disease.Methods:60 Cases of patient were randomly divide into two groups.Both groups were treated by routine medicine and the treatment groups were treated by enema of taohechengqitang 20min with a course of 3days.To observed the changes of the major clinical symptom,lung function,blood routine,blood gas analysis,CRP in both groups.Results:The total efficiency rate of treatment of two groups has significant difference(P<0.05) while the total effective rate has no difference(P>0.05).The effect on treating group is significantly better than the control group(P<0.05).There were high significant differences in the two groups before and after treatment on the improvement of coughing, expectoration,short breath,abnormal defecation situation and wheezing rale (P<0.05),especially in short breath(P<0.01),and there has no statistical significance on the improvement of chest distress and chest pain(P>0.05).The effect of treatment group in cough,expectoration,wheezing shortness of breath,chest tightness,chest pain,abnormal stools and wheezing sound has significant difference(P<0.01).The treatment of the symptoms of coughing, wheezing shortness of breath and wheezing sound has obvious effect in control group(P<0.05),but the improvement on the symptoms of expectoration,chest tightness,chest pain and abnormal stools was not obvious.On the improvement of Blood Gas Analysis,there was no significant difference between two groups in pH(P>0.05),but there has difference between two groups after treatment. After the treatment,WBC and CRP were decline in both groups,and the difference between the two groups was significant(P<0.05).Lung and kidney function and electrolyte(K⁺,Na⁺,Cl^-)were not significant(P>0.05),therefore treated by TCM has no side effect.The lung function indexes were improved after the treatment in treatment groups(P<0.05) inFVC/Pre(%),FVC,but in VC,FEV₁,FEV₁/Pre(%),and VC/Pre(%)the difference between the two groups has no significant(P>0.05).Conclusion:Treating syndrome of phlegm and blood stasis obstructing lung Chronic Blocking Lung Disease by taohechengqitang enema in the theory of through exterior yang organs to treat interior yin organs can improve the clinical symptom of patient,increased the PaO₂,improve carbon dioxide retention,prevent lung infection,shorten the acute exacerbation course with no electrolyte disorder or impairment of renal function.The results showed that the effect of the treated group was much superior to that of the control group.
