Objective: To compare the profiles of serum proteomics across different groups(liver cirrhosis, normal controls and patients classified as Child-Pugh grades A,B and C),aiming at identifying biomarkers; to investigate whether LGT(lost goodwill target) proteome express in cirrhosis or not and analyze the clinical significance of LGT proteome.Methods: Serum protein fingerprint was obtained among subjects with or without cirrhosis using PBS-Ⅱand protein chip (CipherGen, USA) and analyzed with Biomarker Wizard to screen for differences in the expression of protein peaks between 64 patients with cirrhosis(of which 28 of grade A, 19 of B, 17 of C) who cured in Shanxi Provincial People's Hospital from August, 2007 to June, 2008and 20 healthy volunteers in out-patient clinic whose gender and age are matches. We establish the positive diagnosis criterion as following characteristics:having a mass cluster in the range of M/Z values 11100+H~11900+H, according to the dynamic change of LGT proteome, the cases divided into 4 groups and observe the mortality.Results:1,Screening identifying biomarkers in cirrhosis116 protein peak were detected within molecular weight 2000-20000 Da, through analysis found that 16 peaks have significant differences, these proteins are low expression in cirrhosis group (P <0.01).2,The difference protein expression of A, B, C-class with cirrhosis With comparative study the serum protein spectrum about cirrhosis of the Child-Pugh grades A, B, C, found that the protein molecular weight to 8696.97 low expression in Child-Pugh grades B than A. But not between Chilc-Pugh grades B and C.Comparative analysis the Child-Pugh grades A and C, found that 9 significant differences, these 9 proteins are in a low expression in C(P <0.01).3,Searching the difference proteins in database,the most closest to the molecular weight of 8696.97 is apolipoprotein A-Ⅱ.4,The LGT proteome was expressed in the serum of the cirrhosis.The abundance of LGT proteome of cirrhosis group are significantly higher than control(P <0.001), and Child-Pugh grades C higher than A (P <0.05). The death group higher than the survival (P <0.001).5,To observation the dynamic change of LGT proteome, LGT proteome continued positive mortality rate was 86.67%; continued negative mortality rate was 5.56%; from positive to negative was 9.52%; from negative to positive was 30.00%.6,To merge the data between LGT continued positive and from negative to positive, between continued negative and from positive to negative, its chi-square test results showed significant differences(X2 = 30.371, P < 0.001).Conclusion1,Significant differences in a number of proteins were shown between the cirrhosis group and the control group(16 types), able to conduct first diagnosis of patients with cirrhosis.2,A protein low expression in Child-Pugh grades B compared to A, this protein may be apolipoprotein A-Ⅱ. 9 different proteins expression was low in C. But no significant difference between Child-Pugh grades B and C.3,The LGT proteome was exsited in the serum of the cirrhosis, continued positive expression would endanger the lives of the patients; continued negative expression can implied patients in stable condition; from positive to negative implied the condition improved; from negative to positive implied the condition deteriorated. Therefore, as a clinical diagnostic platform to predict disease progression and the prognosis have important clinical significance, provide a new way of thinking for the treatment of the improvement.
