Objective:Acute pancreatitis patients infecting complication easily is an important reason of unfavourable prognosis. Eutrophil leucocyte is the first defense mechanism of organism to resist foreign erodent.The research 1. through determing the expressions of adhering molecule CD11b,CD18 to approach the reason of acute pancreatitis that are easy to infect complication; 2. through utilizing interleukin-2 culture eutrophil leucocyte in vitro, to study and research therapeutical effect of interleukin-2 to eutrophil leucocyte function.Methods:The patients who are divided to MAP and SAP groups according to the standard of SAP diagnosing and concentrating companion which were recommended in hol-countrypancreatic gland surgery seminar in 2006. Each groups elect sufficient twenty examples. Patients are drew-off peripheral venous blood to progress index mapping in the first,third and seventh day after hospital admission. Meanwhile electing Symptomless cholecystolithiasis,Hernia or thyroid innocence disease as control group. Besides utilizing interleukin-2 culture eutrophil leucocyte in vitro two hours which origining from SAP group, after watering bath fifteen minutes to progress index mapping.The expressions determination of adhering molecule CD11b, CD18 : used by flocytometry; functional examination of neutrophilic leukocyte to swallow and sterilize through dihydrogen rhodamine(DHR).Apply SPSS10.5 statistics software to analysis data. All data(x-±s) are used to express, all groups adopt one-factor analysis of variance of measurement data , to compare by applying LSD method, to employ rank sum test when it is heterogeneity of variance, difference have.Results:1 The change of neutrophilic leukocyte adhering molecule CD11b,CD18 in acute pancreatitis1.1MAP:The first day the expressions of neutrophilic leukocyte adhering molecule CD11b,CD18 are more obviously heighten than control group; The CD11b,CD18 metered value in the third and seventh have no statistical significance with the first day, each index appear obviously recuperate.1.2SAP: The expressions of neutrophilic leukocyte adhering molecule CD11b,CD18 in the first and third day are more obviously heighten than control and MAP group,having statistical significance( P <0.05); The CD11b,CD18 metered value in the seventh are more lower than the first and third day, each index appear obviously recuperate, have no statistical significance. 2 Englobement disinfection function of neutrophilic leukocyte in acute pancreatitis2.1MAP: The first day the expressions of DHR are more lower than control group;The third and seventh day are more obviously heighten than control group,having statistical significance( P <0.05); the expressions of DHR in the third and seventh day are more obviously heighten thanthe first day, the seventh day are more obviously heighten than the third day, having statistical significance( P <0.05).2.2SAP: The expressions of DHR are more lower than control group in the first,third and seventh day;The third day are more lower than control group,having statistical significance( P <0.05). The expressions of DHR in the third and seventh day are more lower than the first day, the seventh day are more lower than the third day,having statistical significance( P <0.05).2.3Change after interleukin-2 intervention: The expressions of DHR In the first,third and seventh day is more obviously heighten than SAP group,having statistical significance( P <0.05).3 The correlation about the expressions of CD11b,CD18 and the englobement disinfection functionThe expressions of neutrophilic leukocyte adhering molecule CD11b,CD18 in acute pancreatitis peripheral blood have no dependablity with the englobement disinfection function of neutrophilic leukocyte.But the more the expressions of CD11b,CD18, the low the englobement disinfection function in the earlier period morbidity.Conclusion:1.The expressions of neutrophilic leukocyte adhering molecule CD11b,CD18 in acute pancreatitis peripheral blood have no straight line relationship with the englobement disinfection function of neutrophilic leukocyte.2.Immunization function is not yet suffer serious damage in MAPgroup, organism can snap back steady state by regulation.but the englobement disinfection function of neutrophilic leukocyte continue decreasing in earlier period morbidity in SAP group, existing cellular immune dysfunction.3. Though utilizing interleukin-2 culture eutrophil leucocyte in vitro ,interleukin-2 can elevate the bacteriocidal function of the neutrophils in peripheral blood , improving immune functio of SAP patients.
