Objective: To analyze clinical features of giant vascular malformation in oral and maxillofacial region and methods of diagnosis and treatments for giant vascular malformation. To discuss the importance of radiotherapy in the treatment of giant vascular malformation.Method: Six patients of giant vascular malformation in oral and maxillofacial region which have been treated by 3 dimensional radiation therapy in the department of oral and maxillofacial surgery of the second affiliated hospital of Zhejiang University in the last ten years were reviewed. Postoperative follow up was 6 months to 3 years by correspondence, telephone or examination. The follow-up interval was 6 months to one year. The treatment results were evaluated by clinic examination and imageology results.Results: Dental ulcer was observed in 3 cases during the radiotherapy. No manifest complication was observed in the other cases during the radiotherapy. During 6 months to 9 years follow-up period, temporary dry mouth, pharyngalgia and leucocytopenia were observed in 6 cases, which were cured by time. The contours were all satisfied in all patients. The lesion regions in MRI were all minimized fibrosised and even disappeared. The total effective rate was 100%. There was no sign of recurrence during the follow-up period except a little recurrence which was cut by surgery.Conclusion: 1 Radiotherapy can be a no-wound, safe and effective therapy for oral and maxillofical vascular maiformation. It not only can be the first choice for oral and maxillofical vascular maiformation, but also can be the additional therapy for recurrent cases which were treated by surgery.2 Radiotherapy has an effective result with venous malformation, and it has less effective result with arteriovenous malformation because of its arteriovenous branch and high blood flow rate.2 Considering the deep vascular malformation, the checks of MRI should be done before the operation to diagnose definitely, to evaluate the extent of the lesions, difficulty, review after radiotherapy.3 The therapy for oral and maxillofical vascular malformation remains a combined therapy4 The type of rays, dose, definite effect and long-term curative effect need further research.
