Object:To applicate emodin for intervening the injure of intestinal mucosal barrier during experimental severe acute pancreatitis(SAP) of rats and investgate the protective role of emodin in the intestinal mucosal barrier of SAP rats.Methods:Sixty male S.D.rats were randomly divided into SAP model group(n=20),emodin therpy group(n=20) and sham operation group(n=20).SAP group and emodin therpy group were induced by injection of 3%sodium taurocholate into biliopancreatic duct. The sham operation group was only cut the belly open and touched the internal organs in the rats.The emodin therpy group was injected emodin solution(0.5ml/100g,5mg/ml) into jejunum after modeling success.SAP model group and sham operation group were injected equivalence NS.Twenty four hours after operation,blood was collected from inferior vena cava for detecting serum amylase,serocym endotoxin,serum leptin; pancreatic and 2 cm long ileal tissues that distant from ileocecal about 3 cm were taken for histopathologic examination and apoptosis assay:1) The apoptotic index by terminal deoxynucleotidyl transferase-mediated dUTP nick end labeling(TUNEL);2) the positive rate of Bax protein expression by peroxidase method.Similar ileal tissue was detected by ultramicrostructure in electronmicroscope.Mortality rats were observed also.Results:Serum amylase in SAP model group,emodin therpy group and sham operation group were(3161.50±913.77),(2420.89±559.65)and(361.75±105.46)(U/L,P<0.01) respectively;Serocym endotoxin in SAP model group,emodin therpy group and sham operation group were(170.88±19.39),(104.22±23.21) and(47.80±16.15)(ng/L,P<0.01) respectively;serum leptin in SAP model group,emodin therpy group and sham operation group were(2.91±0.38),(1.84±0.24) and(1.27±0.17)(ng/ml,P<0.01) respectively.Visible inflammatory reaction occurred in SAP model group and emodin therpy group,while there was a little in sham operation group.Pathological changes and ultramicrostructure injurys of terminal ileum in SAP model group were more severe than those in emodin therpy group.The apoptotic index of terminal ileum epithelium in SAP model group,emodin therpy group and sham operation group were(29.64±7.21),(10.37±6.60) and(3.49±3.10)(%,P<0.01) respectively,and the positive rate of Bax protein expression were(55.37±14.35),(41.18±9.31) and(4.28±1.44)(%,P<0.01) respectively.There was significant difference between the motality rate of SAP model group and emodin therpy group(20%and 10%respectively)(P<0.05).Conclusion:Emodin jejunal administration could alleviate the injury of intestinal mucosal barrier and down-regulate intestinal epithelium apoptosis during experimental SAP of rats which may be a protective mechanism of intestinal mucosal barrier,then improve the prognosis of SAP rats.
