Objective:To observe the clinical effect of treating chronic renal failure using the methods of invigorating spleen and kidney, purging turbid and its protective effect on renal function and structure of rats with renal interstitial fibrosis caused by unilateral ureteral obstruction. To discuss its mechanism of function.Methods:1. 120 patients with CRF were divided randomly into two groups with the ratio of 1:1, the experimental group and the control group. Patients in the experimental group (30 cases) were administered with Qudu granules (dispel toxic) (5g per time, three times a day), while those in the control group(60 cases ) treated with Niao Duqing granules (5g per time, three times a day). The changes of hemoglobin, urinary protein per 24hrs,hemoglobin,renal function,parathyroid hormone(PTH),N-actely-β-D-glucosaminidase (NAG), endothelin (ET), changes of scores of symptoms of syndromes of deficiency of spleen and kidney, turbid and blood stasis were observed in two groups. The differences between two groups were compared. 2. 90 male Wistar rats were divided randomly into five groups: the sham operation group,the model group,high dosage Qudu granules group (high dosage group), the low-dosage Qudu granules group (low dosage group) and Niao Duqing group (control group). 18 rats were in each group. Except the rats in the sham operation group , the left ureterals of rats were ligated in each group. Medicine was given to each group according to the designed concentration and dosage from the day before the ligation. The rats were killed on the third, seventh and fourtheenth day in batches. NAG enzyme in the urine, renal function and pathological changes of the kidney were observed. Results:1. After two months treatment, the creatinine, urea nitrogen and uric acid of the patients were improved, NAG enzyme in the urine, endothelin and 24 hours urinary protein were lower, hemoglobin was significantly higher, PTH and scores of symptoms decrease obviously in the experimental group.2. Qudu granules could significantly reduce urinary NAG enzyme, improve the creatinine, urea nitrogen, down-regulate the expression of CTGF in the renal tissue, inhabit the ColⅠin renal tissue, increase the expression of the anti-fibrosis factors HGF,BMP-7 in the kidney and improve the renal interstitial fibrosis. Conclusion: 1. Qudu granules has a good therapeutic effect for the treatment of CRF, which has the function of reducing renal tubular dysfunction, improving renal function, reducing proteinuria and correcting anemia. 2. Qudu granules has the function of protecting the renal function of rats with renal interstitial fibrosis, reducing the NAG enzyme in urine, the renal pathological damage. Its mechanism may be related with down-regulating the expression of CTGF,ColⅠin the kidney, increasing the expression of the anti-fibrosis factors, such as HGF,BMP-7 in the kidney to delay the process of failure of renal function.
