Objective:To observe the change of serum ROS、SOD、MDA of chronic renal failure patients with damp-heat syndrome,explore oxidative stress in the pathogenesis ofchronic renal failure patients and observe the intervention effect of Qingshen Granule on chronic renal failure patients withoxidative stress.Methods:70 cases of CRF patients of damp-heat syndrome, were randomly divided into control group and treatment group, and normal group(20 cases for the detection of serum ROS、MDA、SOD). During the treatment 10 cases lost follow-up, 4 cases in treatment group, 6 cases in the control group, the actual completion of 60 cases, 31 cases in treatment group, 29 cases in the control group. The treatment group and the control group were given the basic treatment of Western medicine. The treatment group added Qingshen granule, 1 per bag(bag granules 10 g, containing about 34 g crude drug), 3 times a day, two of the drug group were not taking other effects of renal function, treatment for 8 weeks. Respectively, before treatment and 8 weeks after the treatment, serum creatinine(serum creatinine, Scr), ROS,MDA,SOD levels, estimated glomerular filtration rate(e GFR), changes of TCM syndrome integral value and the observed before treatment, after treatment.Results:1.Comparison of clinical curative effect:The total effective rate of clinical effect in the treatment group was 83.87%, which was better than which was 51.72% in the control group,the difference was significant(P<0.05). 2.The total effective rate of syndrome of Traditional Chinese Medicine in the treatment group was 83.87%, which was obviously higher than the control group’s 48.28%, the difference was particularly significant(P<0.01).3.TCM syndrome score comparison:The treatment group and the control group have no significant difference before treatment of TCM syndromescore(P>0.05).After treatment,the treatment group TCM syndrome score was significantly lower than that before treatment(P<0.01);the control group TCM syndromes score compared with before treatment had difference(P<0.05);the treatment group TCM syndrome was decreased significantly compared with the control group(P<0.01). 4.Comparison of Scr and e GFR:The treatment group and the control group have no significant difference before treatment of Scr and e GFR(P>0.05).After treatment, Scr and e GFR of treatment group integral than before treatment were significantly different(P < 0.01),but the control group had no significant difference compared with before treatment(P>0.05),and the treartment group was decreased significantly compared with the control group after treatment(P<0.05).5.Comparison of ROS、MDA and SOD levels:Before treament,the level of ROS and MDA in serum of patients was significantly higher than normal group(P<0.01);the level of SOD in serum of patients was significantly lower than normal group(P<0.01);but two groups had no significant difference(P>0.05).After treament,the level of ROS and MDA in serum of treatment grouo patients reduced(P<0.01); the level of ROS and MDA in serum of control group compared with before treatment, no significant difference(P>0.05); and the treatment group redused significantly compared with the control group(P<0.01).After treament,the level of SOD in serum of treatment grouo patients increased(P<0.01); the level of SOD in serum of control group compared with before treatment, no significant difference(P>0.05); and the treatment group redused significantly compared with the control group(P<0.01).Conclusions:1.The levels of ROS and MDA are elevate,levels of SOD is decrease in CRF patients of damp-heat syndrome, It indicates that CRF patients has oxidative stress.2.Qin Shen particles can improve clinical symptoms of patients and treatment effection.3.Qingshen granule can decrease the levels of ROS and MDA,elevate levels of SOD.It indicates that Qingshen granule can inhibit CRF patients of damp-heat syndromeoxidative stress, and enhance antioxidant capacity, thus delaying renal fibrosis.4.Qin Shen particles used in patients with CRF,before and after treatment,routine blood 、urine and stool test,electrocardiograph test、liver function test were all in thr normal range.Objective:Through the detection of unilateral ureteral obstruction(UUO) induced renal interstitial fibrosis(RIF) in the peripheral blood of rats ROS,MDA and SOD expression level to observe of UUO rat the existence of oxidative stress,and to observe the intervention effect of Qingshen granule.Methods:60 healthy male SD rats of clean grade were randomly divided into five groups: normal group,sham operation group, model group, Qingshen granule group, Candecartan cilexetil group, with 12 rats in each group, the rats to detect BUN, Scr showed no abnormality. After 1 week adaptive feeding, model group,Qingshen granule group and Candecartan cilexetil group were prepared by using UUO in rat model of renal interstitial fibrosis,sham operation group had the same operation path separation ureter but not ligation.After surgery on the first day,began to give medicine. Qingshen granule group with renal clearance granule of the weight of 0.8g/200 g dissolved in 2ml of water by gavage, Candecartan cilexetil group with 0.1mg/200 g weight of Candecartan cilexetil dissolved in 2ml of water by gavage, while the normal group,sham operation group and model group with 2ml warm water by gavage. The group was given once a day, until the 4 weekend. 1.Observation the effect of Qingshen Granule on rats BUN, Scr, and 24-hours urine protein.2.ELISA method to observe serum ROS,MDA and SOD.3.HE and Masson staining for pathological examination.Results:1.Comparison of Scr、BUN and 24-hours urine protein quantitative level:Comparison before pretherapy Scr、BUN and 24-hours urine protein quantitative rats in each group,no Significant difference(P>0.05).After treatment,comparison between the normal group and the sham operation group, the level of Scr、BUN and 24-hours urine protein quantitative in rats was no significant difference(P>0.05).Compared with the normal group and the sham operation group,the level of Scr、BUN and 24-hours urine protein quantitative in the model group、Qin Shen particles group and Candecartan cilexetil group was significantly increased after treatment(P<0.01).Compared with the model group,the level of Scr 、 BUN and 24-hours urine protein quantitative in Qin Shen particles group and Candecartan cilexetil group had significantly reduce after treatmnet(P<0.01).Compared with Candecartan cilexetil group,the level of Scr、BUN and 24-hours urine protein quantitative in Qin Shen particles group had reduce after treatmnet(P<0.05). 2.Comparison of ROS 、 MDA and SOD levels:After treatment,compared between the normal group and the sham operation group, the level of ROS、MDA and SOD in rats was no significant difference(P>0.05).Compared with the normal group and the sham operation group,the level of ROS and MDA in the model group 、 Qin Shen particles group and Candecartan cilexetil group was significantly increased after treatment(P<0.01).Compared with the normal group and the sham operation group,the level of SOD in the model group、Qin Shen particles group and Candecartan cilexetil group was decreased after treatment(P<0.01).Compared with the model group, the level of ROS and MDA in Qin Shen particles group and Candecartan cilexetil group were significantly decreased after treament(P<0.01).Compared with the model group, the level of SOD in Qin Shen particles group and Candecartan cilexetil group were significantly increased after treament(P<0.01).Compared with Candecartan cilexetil group, Qin Shen particles group was Significant decreased after treament(P<0.05).Compared with Candecartan cilexetil group,the level of ROS in Qin Shen particles group was no significant difference after treament(P>0.05).Compared with Candecartan cilexetil group,the level in Qin Shen particles group was no significant difference after treament(P>0.05)Conclusions:1.oxidative stress was involved in the occurrence of renal interstitial fibrosis.2.Qingshen granule can reduce serum creatinine, urea nitrogen level, and 24 h urine protein quantity of renal fibrosis in rats.3.Qingshen granule can significantly reduce the renal tissue of rats with renal fibrosis degree, so as to inhibit the renal fibrosis.4.Qingshen granule can reduce the renal tissue of ROS and MDA,rise the renal tissue of SOD,relieve oxidative stress of kidney and to prevent or mitigate the occurrence and development of renal fibrosis.
