Objective1 To analyze the main characteristics of dry eye symptoms and signs in dry eye group and the controls,and to study the correlation between the symptoms and signs.2 To correlate the changes of tear protein profiles with dry eye and potentially use them as diagnostic biomarkers with 2D-nanoLC-nano-ESI-MS/MS used to analyze tear proteins from both dry eye patients and the controls.Methods1 A total of 243 cases(486 eyes) were recruited from December 2007 to August 2008.The patients were carried out a series of checks separately,visual acuity,tear film interferometer inspection,Schirmer testⅠ(STⅠ),dry eye questionnaire(DEQ), 15 minutes after the anterior segment with slit lamp examination,measurement of tear film break-up time(BUT),and Fluorescein(FL) staining.According to the diagnostic criteria of dry eye,126 cases(252 eyes) of dry eye,117 cases(234 eyes) of the controls were studied.2 12 cases(12 eyes) of dry eye patients and 18 cases(18 eyes) of the controls randomly selected,application of iTRAQ technology with two-dimensional liquid chromatography - nano-electrospray ionization tandem mass spectrometry to analysis tear protein collected by Schirmer strip.To verify the iTRAQ findings,we measuredα-enolase and S100 A4 concentrations in tears from an independent sample group of dry eye patients and the controls using ELISA.Results1 Wrinkly and female are more in dry eye patients than in the controls.2 The most common symptom are dryness,the second one is asthenopia and visual fluctuation in dry eye.3 Dryness,foreign body sensation,asthenopia,eye pain,redness,visual fluctuation,photophobia,heavy eyelid,itch and discomfort were significant different between the dry eye and the controls.4 The grade of conjunctivochalasis,MGD and tear debris,the S I t,BUT,FL staining and the tear film interference images were significant different between dry eye group and the controls1) The feeling of heavy eyelid were associate with gender,the remaining eleven symptoms were not associate with gender.2) It has positive correlation between the foreign body sensation,visual fluctuation and age,while it has strong negative correlation between increased secretions and age,the remaining symptoms were not associate with age.3) It has positive correlation between the increased secretions,tearing and SⅠt, the remaining ten symptoms were not associate with SⅠt.4) It has strong positive correlation between eye pain and BUT,the remaining eleven symptoms were not associate with BUT.5) It has positive correlation between eye redness,foreign body sensation and grade of FL staining;the remaining ten symptoms were not associate with the grade of FL staining.6) The tear film interference images were not associate with dry eye symptoms.7) It has positive correlation between the grade of MGD and photophobia.While it has strong correlation between the dryness,visual fluctuation and the grade of MGD,the remaining nine symptoms were not associate with the grade of MGD.8) It has positive correlation between the grade of conjunctivochalasis and foreign body sensation;the remaining eleven symptoms were not associate with the grade of conjunctivochalasis.6 Using advanced 2D-nano-LC-nano-ESI-MS/MS technologies,a total of 93 proteins were indentified in human tear fluid collected by Schirmer strip.7 Eight tear proteins up-regulated and three tear proteins down-regulated in dry eye group compared with the controls with iTRAQ technology.8 ELISA results showed that the tear concentrations ofα-enolase(t-test,p<0.038) and S 100 A4(t-test,p70.027) were significantly higher in tears collected by Schirmer strip in the dry eye group compared to the controls.Conclusion1 Conjunctivochalasis,MGD,tear debris,SⅠt,BUT,FL staining,tear film interference images are important basis for the diagnosis of dry eye.2 It has poor correlation between dry eye symptoms and signs.3 The tear protein had significant differences between dry eye patients and the controls implied the inflammation,cell apoptosis,and metaplasia in the process of dry eye.
