The first partObjective:To review and compare the outcome of patients with benign prostatic hyperplasia(BPH) undergoing suprapubic transvesical and retropubic prostatectomy.Methods:Suprapubic transvesical prostatectomy was performed in 41 patients of BPH and retropubic prostatectomy carried out for 162 patients.The following variables were assessed:operation time,estimated blood loss and serum rescued time,length of stay.Results:There were no significant differences in either the preoperative and postoperative IPSS or urodynamic parameters in both groups.The chief advantages of the retropubic prostatectomy were less trauma to the urethra,which resulted in significantly shorter catheterzation duration(6.59±1.41d versusl3.73±3.25d) and length of recovery(11.29±5.02d versus 18.62±8.66d),few complications,rapid recovery and preservation of antegrade ejaculation.All these differences were statistically significant compared with suprapubic transvesical methods(P<0.05).Conclusion:The retropubic prostatectomy is believed to be the effective and ideal open prostatectomy,and provided good surgical results in our experience.The second partObjective:Urodynamic study of the patient of neurogrnic bladder with cerebral disease,as well as clear the significance of urodynamic study for patient with cerebral disease.Methods:17 cases of neurogrnic bladder with cerebral disease presented with dysuria,urinary incontinence urodynamic in patients with the above-wide inspection.study were preformed and analysied.Results:Complete urodynamic examination of 17 cases of the whole,the results of urodynamic studies:Patients with cerebral vascular accident in 11 cases, including two cases of emergence of detrusor hyperreflexia,detrusor areflexia in 3 cases,3 cases of low detrusor activity,three cases of normal;two cases of traumatic brain injury patients,of which 2 cases were normal detrusor activity,detrusor -sphincter dyssynergia in 1.Three cases of brain tumors,in which the performance of one cases was detrusor hyperreflexia,detrusor the activities of two cases of low performance.1 cases of Parkinson's disease,showing weak detrusor contraction, bladder detrusor and urethral sphincter dyssynegia,urethral sphincter dysfunction.Conclusion:Patients with cerebral disease present withdifferent forms of bladder and urethra dysfunction and urinary tract,dysuria for the merger,in patients with urinary incontinence,urodynamic analysis carried out is very important.
