Objective To study the effection of L-Arginine on Erythrocyte and its Immune Function in patients in Cardiac Surgery with Cardiopulmonary Bypass.Methods Thirty patients undergine elective valve replacement under CPB were randomly divided into two groups:L-Arginine group(L,n=15) and control group(C,n=15).Patiens with respiratory dysfunction, hepato-renal disease,blood disease,hyperlipoproteinemia or pulmonary hypertension were excluded.In group L patients received L-arginine 200 mg/kg which be added into 5%GS 50 ml through internal jugular vein with microinfusion pump at 99 ml/h speed before skin incision.In control group the patients received 5%GS 50 ml in which added nothing.Blood samples were taken from internal jugular vein before skin incision(t1),30 min after initiation of CPB(t2),at the end of CPB(t3),at the end of operation(t4),and 24h after operation(t5),for determination of plasma malondialdehyde(MDA),free hemoglobin(FHb), red blood cell C3b receptor rosette rate(BRC-C3bRR),red blood cell immune complexes rosette rate(RBC-ICR).Results(1)MDA,FHb,RBC-ICR:No significant differences were found of in both group before skin incision.Levels of those in both groups increased significantly after CPB 30 minutes(P<0.05),and them began to decrease at the end of CPB,but those of group L were lower than those of group C at all periods after beginning of CPB(P<0.05).(2)RBC-C3bRR:RBC- C3bRR in both groups dereased significantly after CPB 30 minutes(P<0.05),and them began to increase at the end of CPB,but RBC-C3bRR of group L were higher than RBC-C3bRR of group C at all periods after beginning of CPB(p<0.05).Conclusion CPB causes damage and immunity depression of red blood cell.L-Arginine may protect red blood cell membrane and its immunity of patient during CPB.
