Effects Of IL-12/18 And HBV Genotypes On Antivirus Therapy Efficacy Of Adefovir Dipivoxil In Chronic Hepatitis B Patients

Objective To explore the effects of IL-12/18 and HBV genotypes on antivirus therapy efficacy of Adefovir dipivoxil in patients with chronic hepatitis B.Methods 28 patients with chronic hepatitisB(CHB) who were treated with Adefovir dipivoxil,were recruited.The antiviral efficacy of Adefovir dipivoxil with all the patients were evaluated at the end of 12 and 24 weeks.The HBV genotypes were analyzed by nested PCR using type-specific primers.The levels of serum HBV DNA replication were determined by fluorescent quantitative PCR.HBV serologic markers,the level of interleukin-12 and interleukin-18 of the patients were also detected by ELISA.The statistical software of SPSS 13.0 was used for statistical processing,logistics regression was used to analyse the impact of the treatment factors.Results①The distributions of HBV genotypes in 28 CHB patients were as following:5 patients were genotype B,accounting for 17.86%,18 patients were genotype C,accounting for 64.29%and 5 patients were both genotype B and C,accounting for 17.86%.GenotypesA,D,E and F were not observed in these patients.There were no statistically significant differences in the chang of HBV DNA load,the normalization of ALT and seroconversion of HBeAg in the patients of genotype B,C and mixed genotype(P＞0.05).②The level of serum Interleukin-12 in complete respond group reached a peak at the end of 12 th weeks,then gradually decreased(P＜0.05).③Serum Interleukin-18 level in complete respond group is higher than in partial response group at different point time(P＜0.05);The level of Interleukin-18 in complete response group gradually decreased after treatment,yet a little change in partial respond group (P＞0.05).Conclusion HBV genotype has no correlation with HBVDNA load,ALT level, the level of serum Interleukin-12 and serum Interleukin-18 level;The major genotype of CHB patients in GuangXi is type C,HBV genotype does not influence the response to Adefovir dipivoxil.Antivirus therapy efficacy of Adefovir dipivoxil in patients with chronic hepatitis B relates with the level of serum Interleukin-18.HBeAg-negative patients are more accessible to complete response than HBeAg-positive patients..
