Objective: To investigate the relationship between the metabolism abnormality of the polyol pathway and apoptosis in diabetic retinal tissue,study the possible couse of DR.To explore the new pathways for treatment of DR.Methods: Select 60 healthy male Wistar rats,which were randomly divided into following groups: control, diabetic, and diabetic treated with epalrestat. The diabetic rats and diabetic rats treated with epalrestat received a single 65mg/kg intaperitoneal injecton of STZ dissolved in 0.1 mol/L sodium citrate buffer(Ph 4.5)alone. The control rats received the same sodium citrate buffer. Epalrestat was given from the onset of diabetes at a dose of 100mg / kg·day mixed with the powdered diet.Value of urinary glucose, body weight,FBG and HbA1C were recorded every one week. At the 4th,8th,12th and 16th weeks after model was succeeded,5 rats of each group were anesthetized and measured values of FBG and HbA1C.Then,retinal tissue was obtained, the activity of AR was detected with fluorometric method,apoptosis of retinal microvascular cells was detected with ELISA technique,fix two eyeballs and make retinal vessels stretched preparation to observe the retinal microvascular morphology. Results: 1. After 4 weeks, the diabetic rats showed lower body weight and higher FBG and HbA1C levels than the control rats(p<0.05). Throughout all facets of the study, the diabetic rats exhibited elevation of there FBG and HbA1C. The FBG and HbA1C of the diabetic rats treated with epalrestat compared with control group have no significant deviation(p>0.05).2.The results of ELISA showed that with the development of diabetes, apoptosis of retinal microvascular cells was increasing, contrast with control group has significant deviation(p<0.05),that of the diabetic rats treated with epalrestat compared with control group has no significant deviation(p>0.05).3. During the experimental period,the activity of AR in the retinal of diabetic rats increased compared with those in the control rats(p<0.05),and the activity of AR in the retinal of diabetic rats treated with epalrestat compared with control rats has no significant deviation(p>0.05).4.With the development of diabetes, retinal vessels stretched preparation showed there were disorders and cells reduced,and have great correlation with FBG, the retinal microvascular morphology in diabetic rats treated with epalrestat compared with control rats has no significant deviation.5. The correlation analysis showed that the activity of AR and the levels of apoptosis in retinal of diabetic rats were positive correlation with pathogenesis,the levels of apoptosis in retinal tissue of diabetic rats were positive correlation with FBG, HbA1C and the activity of AR in retinal tissue.Conclusions:1. The ARIs epalrestat prevented microvascular cells apoptosis, prevented the microvascular morphological degeneration,delayed the development of DR.2. The levels of apoptosis in retinal tissue of diabetic rats were positive correlation with FBG, HbA1C and the activity of AR.It showed that the levels of apoptosis in retinal tissue were promoted by the abnormity of glucose metabolism,increasing the activity of AR.3. With the development of diabetes mellitus, the abnormal of retinal capillaries was apparent.and apoptosis of retinal microvascular cells was the early stage change ,then the change of morphology took place.
