Clinical Research On 270 Cases Of Female Climacteric Syndrome Of TCM Syndrome Elements

Climacterium is the special physiological period of every women,according to relevant statistics reports,at the age of 45-55,about 90 percent of women have different degree of clinical manifestations.Climacteric syndrome is a group of syndrome around the climacterium caused by functional disorder of plant neurological.Modern medicine with hormone replacement therapy(HRT) in the treatment of the disease, there are potential risk and adverse reactions,in view of traditional Chinese medicine(TCM) with the guidance of conception of holism and Bianzheng Lunzhi,has certain characteristics and advantages,reduce the effects of adverse side.But with the complicated clinical presentations of climacteric syndrome,at present,there are many types of syndrome differentiation,which crisscross cover each other and against the targeted therapy,therefore,it is necessary to perfect the system of TCM syndrome differentiation of climacteric syndrome. Objective:To investigate the characteristic of TCM syndromes and syndrome elements of climacteric syndrome,and the rule of syndromes combinationwith "syndrome element differentiation", to provide certain evidence for the study on standardization of climacteric syndrome differentiation.Methods:Starting with certification from the disease,used epidemiological survey methods,based on "973" program subject -Study on syndrome norms and the system of syndrome differentiation method - Syndrome elements - Clinical Epidemiology cross-sectional survey -Climacteric syndrome clinical information questionnaire,collecting clinical data on 270 cases of climacteric syndrome.Combined with the theory of TCM to syndrome differentiation by professor with high qualification for clinical experience of climacteric syndrome,the data will be entered into the computer by EpiData3.0 software,to statistical processing and analysis with SPSS15.0 software,analysia syndromes distribution characteristics of climacteric syndrome,and on the basis of relevant syndromes to extract climacteric syndrome elements and the diagnosis of syndrome elements, to investigate the law of combination.Results:1.There are 38 syndromes and 583 times in 270 cases of climacteric syndrome,according to frequency,the first eight syndromes is respectively:kidney-gin deficiency(102), liver-Qi stagnation(80),liver and kidney-gin deficiency (65),kidney-Yin and kidney-gang deficiency(51),heart-Qi deficiency(36),failure of normal physiological function between heart and kidney(34),kidney-Yang deficiency(27) and Qi deficiency(19),total 414 times,occupied 71.01%.2.According to the 38 syndromes extracted 15 syndrome elements:thekidney,liver,heart,spleen(stomach),gallbladder, lung,Yin deficiency,Qi deficiency,Qi stagnation,Yang deficiency and blood deficiency,phlegmy-damp,blood stasis,hot,Yang hyper.Including 4 statistically significant syndrome elements of position:the kidney,liver,heart,spleen (stomach);and 5 statistically significant syndrome elements of nature:Yin deficiency,Qi stagnation,Qi deficiency, Yang deficiency and blood deficiency.3.9 syndrome elements can be obtained by 17 kinds of syndromes combinations,which the form of single syndrome element is 1 kind:blood deficiency;the form of double syndrome elements is 7 kinds:kidney + Yin deficiency,kidney + Yang deficiency,kidney + Qi deficiency,liver + stagnation,spleen + Qi deficiency,heart + Qi deficiency,liver + blood deficiency; the form of three syndrome elements is 6 kinds:kidney + liver + Yin deficiency,kidney + Yin deficiency + Yang deficiency, kidney + heart + Yin deficiency,kidney + spleen + Yang deficiency,kidney + Qi deficiency + Yin deficiency,liver + stagnation + blood deficiency;the form of four syndrome elements is 3 kinds:kidney + Yin deficiency + liver + stagnation,heart + blood deficiency + spleen + Qi deficiency, liver + stagnation + spleen + Qi deficiency;which covers 261 cases of climacteric syndrome,the medical clinic accuracy is 96.67%,the clinical coincidence rate is good.Conclusion: 1.The position of climacteric syndrome more involving the kidney and the liver,the nature mainly is Yin deficiency,and the featuer of pathogenesis is Ben deficiency and Biao excess.2.Syndrome element "Kidney" + "Yin deficiency" and with other syndrome elements is the main form of combination in climacteric syndrome.3.The new system of "syndrome element differentiation" established by the "syndrome element,combinations of syndrome",it is propitious to guide the clinical syndrome differentiation,and the standardization of TCM syndrome differentiation is feasible.
