Objective:The incidence and mortality rate of gastric cancer rank first among the malignant tumors in digestive system.In recent medical work,priority should be given to researchs on the epidemiology and the molecular biological etiology and pathogenesis of gastric cancer.Our study discusses the pathogenesis of gastric cancer,which is based on risk factors and the expression of CDK6 in gastric cancer tissue. Firstly,clinical study on the related risk factors of patients with gastric cancer in Tianjin is carried out,in order to provide clues and evidence for the study on etiology and the prevention of gastric cancer.Cyclin-dependent kinase(CDK) has promoting effect on cell differentiation and mitosis.Many kinds of tumors have abnormal expressions,which are related to the staging,metastasis,and prognosis of some of the tumors.Few researchs study the relationship between CDK6 and gastric cancer.Our study detects the expression of CDK6 in gastric cancer tissue,discusses the relationship between CDK6 and gastric cancer in clinical and pathological features,and understands its effect on the occurence and development of gastric cancer.Method:1.Epidemiological investigation of 100 patients with gastric cancer in Tianjin, receiving gastroscopy or surgery,diagnosed by pathological examination,were carried out,and 109 cases of chronic superficial gastritis and chronic atrophic gastritis contemporarily diagnosed by gastroscopy were set as normal controls.And Logistic regression analysis of the epidemiological investigation result of the risk factors of gastric cancer is made,in order to discuss the relationship between the occurrence of gastric cancer and risk factors such as diet,emotion and living habits.2.Samples of the 86 cases were divided into 3 groups:26 cases of gastric cancer tissues(patients without pre-operative/pre-biopsy radiotherapy or chemotherapy),30 cases of precancerous lesion,and 30 cases of normal gastric tissues. Immunohistochemical SP method was applied to detect the expression of CDK6 in tissues.Analyzed the expression of CDK6 in the tissues in group of gastric cancer,group of precancerous lesion,and group of normal controls.And analyzed the differential expression of CDK6 in gastric cancer and its relationship with gastric cancer in clinical and pathological features.Result:1.Smoking,alcohol,tea and a family history of malignant rumor will lead to a significant increase of the risk of developing gastric cancer.Smoking more than 20 cigarettes per day can increase the risk of gastric cancers to 3.386 times the risk for a nonsmoker,while this number is 10.202 for alcohol,3.352 for tea and 3.356 for a family history of malignant tumor accordingly.2.The positive rate of CDK6 expression in gastric cancer tissue,in precancerous lesion group,and in normal controls is 73.08%(19/26),23.33%(7/30),and 10% (3/30) respectively.The positive rate of CDK6 expression shows significant difference between gastric cancer tissue,and precancerous tissue and normal tissue. (respectively X~2 13.857,23.234,p<0.05);The precancerous tissue and normal tissue show no significant difference(X~2=1.92,P>0.05).The expression intensity of CDK6 in gastric cancer tissue and normal tissue show significant difference(Uc=4.59,P<0.05).The positive rate and expression intensity of CDK6 in gastric cancer tissue have no obvious correlation with clinical and pathological features such as patients' age,tumor diameter,invasive depth,pathological differentiation degree and lymphatic metastasis(P>0.05).Conclusion:1.Smoking,alcohol,tea and family history of malignant tumor will all greatly increase the risk of having gastric cancer.2.The abnormal expression of CDK6 in gastric cancer tissue breaks the balance of regulatory system of cell cycle,leads to uncontrolled cell cycle regulation,and causes unlimited and independent multiple fission,thus resulting in canceration,which is possibly significant in the occurrence of gastric cancer.3.The positive rate of CDK6 expression tends to increase gradually from normal controls,to precancerou lesion group,and to gastric cancer group,indicating that it may be a kind of early event,which may be applied as an early diagnostic marker.4.The expression of CDK6 has no obvious correlation with tumor diameter,invasive depth,pathological differentiation degree and lymphatic metastasis, in the whole process of the occurrence and development of gastric cancer,indicating no correlation with development and invasive ability of gastric cancer.
