Objective:To investigate the influence of Hydroxyethyl Starch 130/0.4 on fluid balance;metabolism of natrium;hemodynamics;APACHEⅡscores and pulmonary function after major abdominal surgery.Methods:60 patients undergoing elective major abdominal surgeries were selectd and devided into two groups,crystalloid group and Hydroxyethyl Starch(HES)group. Patients in crystalloid group were just administrated with crystalloid.That in HES group,additional 500ml HES 130/0.4 was given for consecutive 3 days.The changes of fluid and natrium balance;hemodynamics;APACHEⅡscores were recorded on the operation and post opration days.Forced vital capacity(FVC),forced expiratory capacity in first second(FEV1.0),maximal voluntary ventilation(MVV),oxygenation index(PaO2/FiO2),respiratory index(RI) were measured by lung function equipment and blood gas analysis pre and post opration days.Results:The positive fluid balance of crystalloid group was significantly more than that in HES group on postoperative day 1,2,3(P<0.05).The hemodynamics of HES group was significantly better than that in crystalloid group on postoperative day 1,2(P<0.05).The APACHEⅡscores of crystalloid group were higher than those in HES group on postoperative day 2,3(2<0.05).FVC,FEV1.0,MVV increased significantly in HES group more than those in crystalloid group on postoperative day 2,3(P<0.05). The alteration of PaO2/FiO2,RI in HES group were significantly more than those in crystalloid group on postoperative day 1,2,3(P<0.05).Conclusion:The positive fluid balance and natrium retention happened after the major abdominal surgery.The positive fluid balance and natrium retention decreased with the application of HES 130/0.4.It was also able to decrease the APACHEⅡscores,promote pneumonic function recovery and diminish lung injury.It means that the application of HES 130/0.4 can promote the recovery of the patients undergoing major abdominal surgery.
