Effect On Acupuncture Therapy Of "Tong Tiao Du Ren" In Initial Post-Stroke De Pression

Purpose:To observe the effects of "tong tiao du ren" acupuncture therapy on initial post-stroke depression(PSD) through observation of clinical symptoms and evaluation of activities of daily living,analyze the target factors,and offer practicable acupuncture treatment.Method:1.Based on the strict diagnostic criteria,the exclusive criteria,inclusive criteria,and evaluating criteria,40 patients of initial PSD selected from neurology department,the acupuncture and moxibustion department,and rehabilitation center of Quanzhou hospital of Chinese medicine were randomly divided into the "tong tiao du ren" acupuncture group and the control group,20 patients in each group.2.The "tong tiao du ren" acupuncture group were treated by acupuncture on the main points Houxi(SI3) which connects with DU meridian,Lieque(LU7) which connects with REN meridian,Baihui(DU20) and Yintang(EX-HN3),with modified matching acupoints according to different syndromes,once a day;The control group took Fluoxetine hydrochloride(trade name:Prozac) 20mg at about 8 every morning,Both treating courses consisted of four weeks.3.Both groups were evaluated by Hamilton Depression Scale(HAMD) 24-item version and Barthel Index(BI) before and after the treatment,and three months later after the treatment, all patients would be evaluated by HAMD again.Results:1.After the treatment,the scores of HAMD in both groups were decreased significantly(P＜0.01,P＜0.01),indicating that both treatments improved the symptoms of depression significantly,but no significant difference presented between the two groups(P＞0.05), indicating that both the treatment of acupuncture and that of Prozac had the antidepressant action.2.The "tong tiao du ren" acupuncture group's treatment showed better effect than the control group's on the factors of anxiety/somatization,sleep disorder and desperation(P＜0.05).3.After the treatment,the scores of BI of both groups were increased significantly(P＜0.01, P＜0.01),indicating that both treatments improve ADL.And the scores of BI after the treatment were different(P＜0.05).But the added scores of BI in the "tong tiao du ren" acupuncture group were significantly higher than that in control group(P＜0.01),indicating that the "tong tiao du ren" acupuncture group's treatment was better effect than the control group's on improving ADL.4.HAMD scores of both groups have no significant difference between after the treatment and three months later after they were treated(P＞0.05,P＞0.05).Three months later,HAMD scores have no difference between the two groups(P＞0.05 ),indicating that the antidepressant effect of the "tong tiao du ren" acupuncture group was sustained to some extent.Conclusion:1.The acupuncture therapy of "tong tiao du ren" is effective on symptoms of initial post-stroke depression,and it is superior to Fluoxetine hydrochloride on factors of anxiety/somatization,sleep disorder and desperation especially.2.The acupuncture therapy of "tong tiao du ren" can not only be the favorable approach in adjusting PSD,but also improve activities of daily living.
