Objective:Stroke-associated pneumonia is common complications for stroking patient and malnutrition is pervasive phenomenon for inpatient. This investigation aims at delving into relativity of risk factors between malnutrition and SAP.This study can guide guarding against stroke-asso- ciated pneumonia as well as enhancing cognition for the old malnutrition in clinic.Methods: We have done prospective analysis for initial incidence of old cerebral ischemic stroke patients in the Department of Neurology of First Affiliated Hospital of Dalian Medical University during the period of from January 1st 2008 to December 30th 2008.The investigation mainly includes following parameters:basal state(Sex, smoking history,diabetes history, hypertension history,.hyperlipidemia, heart disease),some factors of stroke-related (the block volume, category of cerebral stroke by the type standard OCSP).Within 24-hours's an evaluation of patient's nutritional status (including body mass index,BMI; triceps skin-fold thickness,TSF; serum albumin, sALB; mid arm muscle circumference,MAMC).All patient have been observed for 2 weeks, monitoring whether to be attacked by SAP, All patients were divided into 2 groups by whether patient went down with SAP, and incidence rate of SAP was ascertained chi square test. Through factor analysis, selected factor(Ñ€< 0.05) compared with many factors, returning to logistic analysis.Results:1,210 chosen-patients consist of 71 pneumonia and 139 no-pneumonia, in 71 Stroke-associated pneumonia patients cover 45 early-onset pneumonia patients and 26 late-onset pneumonia patients. (Percentage: 63% versus 37%.) 2,Sixty-five patients were diagnosed in malnutrition status in 210 stroke patients, percentage of sickness was 31% . There were 21(32%) protein type malnutritions, and 32(49%) protein-energy malnutrition patien- ts and 12(19%) mixed type malnutrition patients.3,Thirty-six of malnutritional patients occoured SAP. There were 22 early-onset pneumonia patients,eleven of them were protein-energy malnu- trition,protein type malnutritions were four patients and seven mixed type malnutrition patients; There were 14 late-onset pneumonia patients, three of them were protein type malnutritions, protein-energy malnutrition were nine patients and two mixed type malnutrition patients.4,Through one factor analysis,comparing pneumonia patients with no-pneumonia patients in diabetes history,smoking history,the block volume,malnutrition,we found the value of survey in Statistics.5,Through many factors analysis,incidence rate of pneumonia of stroke patients who have diabetes history is 6.715 as high as who have no diabetes history.Probability of catching pneumonia to patients who have smoking history is 5.547 times as high as who have no smoking history. Patients who have large infarct volume is 7.505 times as high as who have smaller infarct volume in sickness rate of pneumonia.Case rate of pneu- monia to malnutritional patients is 12.144 times as high as patients with a good nutritional state.Conclusions:1,Sickness rate of malnutrition especially protein-energy malnutrition is high for the old ischemic stroke Inpatients.2,Stroke-associated pneumonia has high sickness rate in malnourished Inpatients.3,Malnutrition is one of the risk factor for cerebral ischemic stroke patients.
