Objectives: To explore the risk factors of stroke associated pneumonia(SAP)in different classification of acute cerebral infarction,providing information for clinical prevention.Methods: Consecutive acute cerebral infarction patients who were admitted to Department of Neurology,The People's Hospital of Guangxi Zhuang Autonomous Region were prospectively recruited from January2015 to March 2017.The baseline data includ age,sex,hypertention,diabetes,atrial fibrillation,previous lung disease(including pulmonary atelectasis,pulmonary emphysema,chronic obstructive pulmonary disease),the National Institute of Health Stroke Scale(NIHSS)scores,blood lipid level,blood albumin,the classification of OxfordshireCommunity Stroke Project(OCSP: total anterior circulation infarct,partial anterior circulation infarct,posterior circulation infarct and lacunar infarct),swallowing function,state of consciousness,trachea cannula or not,gastric intubation or not,pneumonia or not,etc.After baseline dats were recorded,calculate the rate of stroke associated pneumonia in different classification of OCSP.Single and Multiple Logistic regression analysis,Kaplan-Meier curve and log rank test were used to analyze.Results:1.In a total of 804 patients,the rate of SAP were 9.8%;in the classification of TACI(66patients),the rate of SAP were 48.5%;in the classification of PACI(281patients),the rate of SAP were 11.7%;in the classification of LACI(315patients),the rate of SAP were 3.5%;in the classification of POCI(142patients),the rate of SAP were 12.7%.2.Single factor analysis shows that age,diabetes,hypoproteinemia,dyphagia,disturbance of consciousness,high score of NIHSS,trachea cannula and gastric intubation were related to SAP in classification of TACI patients;Age,sex(male),smoking history,previous lung disease,hypoproteinemia,dyphagia,disturbance of consciousness,trachea cannula and gastric intubation were related to SAP in classification of PACI patients;Age,hypoproteinemia,disturbance of consciousness,were related to SAP in classification of LACI patients;hypoproteinemia,dyphagia,disturbance of consciousness,high score of NIHSS,tracheacannula and gastric intubation were related to SAP in classification of POCI patient.3.Multiple Logistic regression analysis shows that previous lung disease and hypoproteinemia were risk factors of SAP for patients in classification of PACI,and that hypoproteinemia were risk factor of SAP for patients in classification of LACI.Conclusion: Previous lung disease and hypoproteinemia were risk factors of SAP for patients in classification of PACI,and that hypoproteinemia were risk factor of SAP for patients in classification of LACI.
