Objective To study the effects of the donor nerve pretreatment using Tripterygium glycoside(TG) and the recipient peroral TG after operation ,on the skeletal muscle apoptosis of the recipient after sciatic nerve allograft.Methods Forty adult SD male rats were randomly divided into four groups ,with ten rats in each .In Group A , allograft; in Group B, allograft and the recipient peroral TG after operation , 5mgkg-1.d-1; in Group C, allograft ,only the donor sciatic nerve pretreatment using TG in advance , and the recipient peroral TG after operation , 2.5mgkg-1.d-1 ; in Group D , allograft ,only the donor sciatic nerve pretreatment using TG in advance , and the recipient peroral TG after operation , 5.0mgkg-1.d-1;in Group E, allograft ,only the donor sciatic nerve pretreatment using MEM in advance , and the recipient peroral TG after operation , 2.5mgkg-1.d-1.In each groups, the nerve graft was used to repair 10mm extent gap using10-0 nylon tensionless discontinuous epineurium suture. The donor nerves were taken from Wistar male rats. All of the rats begin medication from the second day after operation and lasted five weeks, but the rats were peroral 0.9% NaCl solution in Group A after operation. At 3 weeks and 6 weeks after operation , the tibialis anterior muscles were observed , involving the mass, the diameter, the area , and the rates of Bcl-2,Bax and TUNEL positive staining, the ultrastructure of the skeletal muscle with Electron Microscopy.Results There were no significant difference (P﹥0.05) in the inter-group of Group B,C,D ,and Group A,E ; whereas, there were significant difference between Group B,C,D and Group A,E ( p﹤0.01). It was showed that the skeletal muscles atrophy were more serious in Group A,E than other groups. The microscopy showed that there were different skeletal muscles atrophy in all groups after the denervation, and the muscle fibers became thin and the gap were wide; as time goes on, the skeletal muscles atrophy were much more serious , and the muscle fibers became more thin, and the gap were more wide, and the periodic striated structure were not clear and there were more connective tissue and more nuclei could be observed in Groups A,E, moreover, the other groups recovered gradually. The electron microscopy showed that there were the sarcoplasmic reticulum spread and the muscle silk dissolution and condensation of the mitochondria in the early denervation. At 6 weeks after operation, there were a mass of the muscle silk dissolution and sarcomere damage and fragmentary Z lines in Group A,E, in other groups , the myofibrils and Z lines arranged tidily, they were clear on the light band and the dark band and the sarcomere.Conclusions (1) The immunogenicity of fresh donor nerve was very heavy, and the skeletal muscles atrophy of the recipient with allograft were serious. (2) The recipient was oral TG could prevent the skeletal muscles apoptosis after sciatic nerve allograft. (3) Donor sciatic nerve pretreatment using TG solution could reduce the dosage of immunosuppressant of the recipient after operation. (4) The effects of the donor sciatic nerve pretreatment using TG in advance and the recipient oral TG after allograft are synergistic.
