Objective To introduce a case of renal artery aneurysm treated with ex vivo renal revascularization and autotransplantation,and to discuss it with literatures.Methods A right renal artery aneurysm(11mm×20mm) with acute hypertension(160/110mmHg) located in the middle of the main renal artery was diagnosed by Renal artery angiography.After aneurysmectomy and temporary nephrectomy, renal revascularization were done with the kidney protected by hypothermia and continuous perfusion with preservation solution,and then the kidney was replanted into the right iliac fossa.Results The operation was done successfully and there were no significant perioperative complications. the blood prossure stabilized at about 135/90mmHg basically after the surgery. Although a serum creatinine level temporarily exceeded above 144 umol/L,it recovered gradually within 12 days.color Doppler ultrasonograph demonstrated patent reconstructed renal vascular in the right iliac fossa two weeks.Follow-up 140 days after the operation:the blood prossure 132/92 mmHg, serum creatinine 86 umol/L,Doppler ultrasonograph demonstrated patent reconstructed renal vascular.Conclusion This technique treating the renal artery aneurysm with secondary hypertension is safe and feasible to manage renal artery aneurysm.
