Objective: Study on the difference of monitoring indicators, electrocardiogram and blood pressure between lead workers and non-lead workers; the changes of monitoring indicators, electrocardiogram and blood pressure of the lead poisoning target after lead-refoulement treatment, and relation between changes of electrocardiogram or blood pressure and lead treatment. To further improve the diagnosis level of abnormal ECG and high blood pressure of workers caused by lead, reduce the misdiagnosis occurrence of cardiovascular disease caused by lead.Methods: According to the study inclusion criteria, select the front-line workers in the smelter to be the observed object of study group, the sex and age-matched related administrative support staff as a normal control group. Monitor the blood lead(PbB), zinc porphyrin(ZPP),δ-ALA , electrocardiogram and blood pressure, and compare the differences between two groups. Determine the target of lead poisoning and give them 20% calcium disodium EDTA (EDTA) intravenous drip of lead treatment referring to "occupational chronic lead poisoning diagnostic criteria" (GBZ37-2002), and follow up the monitoring indicators, electrocardiogram and blood pressure in three to six months after finishing treatment, then analyze the relevance of electrocardiogram, blood pressure changes and lead treatment.Result: 1.Comparison of related indicators difference between study group (lead group) and control group (non-lead group): the average level and abnormal rate of the blood lead(PbB), ZPP,δ-ALA is 487.49μg/L(30.50%),14.30μg/gHb(55.00%),7.80mg/L(49.50%),the abnormal rate of electrocardiogram and blood pressure is 44.00%,21.50% in lead group;the average level and abnormal rate of the PbB, ZPP,δ-ALA is 370.72μg/L(6.50%),8.90μg/gHb(25.50%),5.18mg/L(26.50%),the abnormal rate of electrocardiogram and blood pressure is 44.00%,21.50% in non-lead group;2. Comparison of related indicators between before and after EDTA treatment of lead poisoning objects(treatment group): the average level of the blood lead(PbB), ZPP,δ-ALA before treatment is 564.70μg/L,18.20μg/gHb,12.53mg/L;the average level of PbB, ZPP,δ-ALA after treatment is 382.82μg/L,12.10μg/gHb, 8.32mg/L;The abnormal rate of electrocardiogram and blood pressure before treatment is 48.16%,29.84%,while after treatment is 33.50%,15.18%;The differences of all related indicators are statistically significant.3. The impact of electrocardiogram and blood pressure change by EDTA treatment of lead-refoulement: according to the monitoring indicators, different doses of EDTA treatment were used for the lead poisoning objects. EDTA has remedy for abnormal electrocardiogram and high blood pressure. The difference of efficiency of electrocardiogram and blood pressure in different EDTA treatment doses is not statistically significant.Conclusion: The abnormity of blood lead(PbB), zinc porphyrin(ZPP),δ-ALA, electrocardiogram and blood pressure of the study group is significantly higher than that of control group, so lead can cause the level of blood lead (PbB), zinc porphyrin (ZPP),δ-ALA to rise, abnormal electrocardiogram and high blood pressure; EDTA lead-refoulement can reduce the blood lead (PbB), ZPP,δ-ALA values of lead poisoning objects; EDTA have the effectiveness on both abnormal electrocardiogram and hypertension of lead poisoning objects, and there is no difference of the efficiency in different treatment doses of different course.
