PURPOSETo analyze the clinical characteristics, surgical outcomes and final post-operative best corrected visual acuity of pseudophakic retinal detachment. To identify any potential correlations between complications in the cataract surgery and the features of RD as well as the final visual outcome.METHODSRetrospective review of 53 eyes of 53 consecutive patients with RD occurring after cataract surgury and followed for 6 months or more after surgery.RESULTS(1) Twenty out of 53 patients(38%) were high myopia. The mean age at the time of PPV surgery was lower than the patients without high myopia(55.42±10.77s vs 62.48±7.55s).The interval time from cataract to RD of the eyes with posterior capsular rupture is shorter than the one of the eyes with an intact posterior capsule(11.24 vs 14.22).The preoperative detected retinal holes were 39.However,the retinal holes were 111 by the observation during the PPV.The undetection rate of RD holes was 41.5%. The omission factor of RD hole was 26.4%.(2) The overall rate of anatomic reattachment was 90.6%.The relapse rate of RD was 9.4%.The rate of anatomic reattachment of PPV without scleral buckling was 87.5%.The rate of anatomic reattachment of vitrectomy with scleral buckling was 91.9%.(3) The binomial Logistic regression analysis identified the main significant factors for the retinal anatomical reattachment PVR(P=0.0004)(4) A best-corrected visual acuity of 0.3 or better was achieved in 9 out of 53 eyes(17%).Three variables were independently correlated with visual results:high myopia, redetachment associated with the development of PVR,and macular hole.CONCLUSIONS(1) Several risk factors for RD after CE included predisposing preoperative factors unrelated to the surgical, procedure(myopia, young age),intraoperative complications(vitreous loss, rupture of posterior capsule with or without vitreous loss),and postoperative factors (performance of posterior capsulotomy). The detection rate of RD holes was low.(2) The overall rate of anatomic reattachment of PPV is high. The rate of anatomic reattachment for primary PPV alone and vitrectomy with scleral buckling is similar.(3) The binomial Logistic regression analysis identified the main significant factors for the retinal anatomical reattachment was PVR.(4) Three variables were independently correlated with visual results: high myopia, redetachment associated with the development of PVR,and macular hole.
