Expression Of CD5+CD19+B Cell And IgA1 In IgA Nephropathy Patients' Tonsil And Analysis With Relative Factors

Backgroud:IgA nephropathy is one of the main renal diseases,which induced ESRF.Many researches have demonstrated chronic reccurent tonsillitis has a relation with the pathogenesis and development of IgA nephropathy.Some reports revealed that the abnormal structure of IgA1 productions were increased in the tonsils with IgAN.Recently,sufficent evidences proved that CD5+CD19+Bcell may play a important role in the development of IgA nephropathy.E.g the number of CD5+CD19+B cell was significantly more in the peripheral blood,peritoneal fluid and kidney biopsies than control subjects.However,the relationship between CD5+CD19+Bcell and IgA1 in tonsil with IgA nephropathy has not reported.Objective.To analysis the expression of IgA1 and CD5+CD19+B cells in palatine tonsils of patient with IgA nephropathy.To examine the correlation between the expression of IgA1 and CD5+CD19+Bcell and found out the relationship between CD5+CD19+Bcell and pathological changes.Methods:Eight patients were diagnosed as IgAN via renal biopsy.Eight patients suffering from chronic tonsillitis without nephritis were selected as control.Immunofluorescence were applied in all selected tonsils to observe the expression of IgA1,CD5+CD19+Bcells. All the clinical informations of the research objects were collected: including urinary sediment,renal function,and biosy report.Results:1.Compared to non-nephritis group,the percent of CD5+CD19+B cells in germinal center of tonsils was significantly increased (P＜0.01)2.Compared to non-nephritis group,the percent of IgA1 positive cell in subepithelial of tonsils was significantly increased(P＜0.01)3.There was a significant positive correlation between CD5+CD19+B cells and IgA1(P＜0.05)4.In IgAN,the percent of CD5+CD19+B cell in patients accompanied hematuria with proteinuria group is higher than that of patient with hematuria only(P＜0.05)5.The number of CD5+CD19+Bcell in patients with IgAN≥ⅢLee's grade is higher than that of in＜Ⅲgrade(P＜0.05)Conclusion:1.Compared to non-nephritis group,the number of both IgA1 positive cells and CD5+CD19+B cells increased in tonsils of patients with IgAN(p＜0.01)There was a significant positive correlation between CD5+CD19+B cells and IgA1 positive cells.2.In IgA nephropathy,there is a correlation between the expression of CD5+CD19+Bcells and pathological changes.
