Objective:To study the reference value of serum Gd-IgA1 and its specific antibody IgG anti-Gd-IgA1 and IgA anti-Gd-IgA1 in the diagnosis of IgA nephropathy and the relationship between 24 h urine protein quantitation and pathological grade in IgA patients.Methods: Thirty-seven patients with IgA nephropathy diagnosed by renal biopsy were selected as the experimental group(referred to as IgAN group),and 35 patients with chronic nephritis(non-IgAN group)and 10 patients with non-IgA nephropathy who met the inclusion criteria were selected.In the same period,the normal physical examination patients were normal control group.The general indicators and clinical biochemical indexes were collected.The venous blood was collected separately.After high-speed centrifugation,the upper serum was taken for ELISA to detect serum Gd-IgA1 and IgA anti-Gd-IgA1 and IgG anti-Gd-IgA1 levels.Results:1.The 24 h urine protein level in IgAN group was lower than that in non-IgAN group,and there was significant difference between them(P<0.01);the number of urinary red blood cells in the IgAN group was higher than that in the non-IgAN group,and there was a significant difference between the two groups(P<0.05).2.The Gd-IgA1 in the serum of IgAN group was significantly different from the healthy control group(P<0.01),and there was significant difference compared with the non-IgAN group(P<0.05).The serum IgA anti-Gd-IgA1 in the IgAN group was significantly different from the healthy control group(P<0.05),there was a significant difference(P<0.05)compared with the non-IgAN group.The serum IgG anti-Gd-IgA of IgAN group There was a significant difference compared with the healthy control group(P<0.01),and there was a significant difference compared with the non-IgAN group(P<0.01).3.The levels of Gd-IgA1,IgA anti-Gd-IgA1 and IgG anti-Gd-IgA1 in serum have significant significance for the diagnosis of IgAN,and serum IgG anti-Gd-IgA1 antibody is more diagnostic.Value,sensitivity is 94.6%,specificity is 73.3%.4.Serum IgG anti-Gd-IgA1 antibody levels were positively correlated with 24 h urinary protein quantitation in patients with IgAN(r=0.406,P=0.013),and positively correlated with pathological grade(r=0.408,P = 0.012).Conclusion:1.Gd-IgA1,IgA anti-Gd-IgA1 and IgG anti-Gd-IgA1 levels can be used as non-invasive detection indicators for IgAN diagnosis,wherein IgG anti-Gd-IgA1 antibodies may be more valuable.2.The serum IgG anti-Gd-IgA1 antibody level in IgAN patients was correlated with 24 h urine protein quantitation and Lee's pathological type,suggesting that IgG anti-Gd-IgA1 may have a certain significance in monitoring the clinical progression of IgAN patients.
