Study On Clinical Blood Transfusion In 2004-2008 And Trend Prediction Of Hunan Province

Objectives: On the observation of the state of component transfusion and the changes of clinical transfusion, to analyze the velocity of increase, seasonal change and department distribution, and approach the rationality of transfusion and tendency in the future. We will provide research data to the application of standardized management of clinical transfusion and improve the transfusion. The program of blood bank's work and volunteer donation will be advanced on the scientific basis.Methods: To analyze the amount changes of total blood and blood component according to the data of Blood management office annual report of ministry of health of hunan and Yue-yang blood bank and Changsha blood center from 2004 to 2008 through retrospective essay, then apply the model of ARIMA time series analysis to predict the usage of red blood cell of Yueyang area in 2009. Statistical analyze the transfusion date of 12 different grade hospital in different areas through 2004 to 2008, calculating the composition of component transfusion and distribution in different department, and investigate the rationality of transfusion in 8 hospitals, then applied the x~2 test to compare the difference between different grade hospitals.Results: 1. The proportion of component transfusion is increased from 89.0% in 2004 to 99.7% in 2008. 2. The amount of blood used in clinic increased 49.06% in 5 years, mean increased 9.81% each year. Component blood varied in growth rate, the fastest was platelet, and the following were plasma and red blood cell. The consumption of plasma varied in different areas. 3. The consumption of blood was least in January and February, come into a platform after a great increase in March. The fluctuation of red blood cell consumption is steadier than plasma by the month. 4. Among the various departments of the hospital, department of hematology, neurosurgery and infectious occupied the bulk of blood usage, the proportion of gynecology and pediatric department and other departments retain lower than 15%. 5. The rate of rational transfusion increased from 74.5% in 2004 to 91.5% in 2008, the plasma is the outstanding problem as common. 6. Construction of the model of ARIMA and applied to forecast the consumption of red blood cell in 2009 of Yue-yang, predicted the mean level is about 3000U per month.Conclusions: 1. The ratio of component transfusion is 99.7%, reach to the international standards. 2. The quantity of blood use had a great increase in 5 years lately, and will stay grow by 10%. 3. The clinical transfusion has the seasonal fluctuation, the decline in January and February maybe due to the shortage of provision. 4. Some department occupied the majority of blood use, enhanced the supervision and control of these department is critical to promote rational transfusion and blood save. 5. The rate of rational transfusion reached 91.5% in our province, owned much advantage than other area, but unreasonable transfusion in plasma is common, more attention should be paid. 6. It is feasible to apply the model of ARIMA to predict the blood use in the future, and scientific basis will be provided in work plan decision.
