Effect Of Hemoperfusion Combined With Hemodialysis On Disorders Of Calcium-phosphate Metabolism In Maintenance Hemodialysis Patients

Objective:Two groups of patients who received maintenance hemodialysis(MHD) were given HDP and HD treatment respectively for three months,before and after treatment by measuring blood iPTH,serum phosphate,calcium,calcium and phosphorus product,serum albumin,cholesterol,creatinine,urea and other indicators of the changes,we observed the impact of HDP on the disorders of calcium-phosphate metabolism and nutritional status received MHD patients.Methods:30 maintenance hemodialysis patients were divided into two groups:HDP group and HD group in blood purification treatment centre of our hospital.All the cases were ruled out serious heart,liver disease,or with pregnancy and other serious chronic illnesses or diseases such as severe infection merger。Receive hemodialysis time 13 to 67(average 30.1±10.9) months.After basic therapy about two weeks (including diet control,blood dialysis treatment,the use of erythropoietin and antihypertensive treatment,and so on),Patients were randomly divided into hemodialysis combined with hemoperfusion(HDP) group with 16 cases and hemodialysis(HD) group with 14 cases.HDP Group was given hemoperfusion combined with hemodialysis 2h first,then removed the perfusion,to hemodialysis 2h, HD group received regular blood Dialysis treatment,the treatment time for 4h.Blood iPTH,serum phosphate,calcium,calcium and phosphorus product,serum albumin, cholesterol,creatinine,urea and other indicators of changes in serum were measured before and after the treatment.Blood iPTH were measured by application of radioimmunoassay method while serum calcium,Phosphorus,cholesterol,albumin, creatinine and urea were using automatic biochemical analyzer determination.Datas were analysed by SPSS10.0 statistical software.P<0.05 for the differences were significant,P<0.01 for the notable significant difference.Results:(1) HDP group:Serum iPTH decreased significantly from(799.4±598.1) pg/ml to(517.1±414.4) pg/ml after treatment.Average clearance rate was 34.1%; serum phosphorus decreased significantly from(2.48±0.65) mmol/L to(0.99±0.20) mmol/L after treatment.Average clearance rate was 58%;albumin increased from (30.21±4.98) g/L to(35.52±5.14) g/L,cholesterol increased from(3.21±1.13) mmol/L to(4.39±1.25) mmol/L,both have statistical significance(P<0.05)。(2) HD group:serum phosphorus decreased significantly from(2.43±0.72) mmol/L to(1.03±0.21) mmol/L after treatment,average clearance rate was 53%.Ohters had no statistical significance(P>0.05)。Conclusion:(1)Hemoperfusion associated with hemodialysis can effectively clear blood PTH,but on the contrary hemodialysis cannot.(2) Not only HDP but also HD can eliminate serum phosphorus significantly after single dialysis.(3) HDP Group blood calcium and phosphorus product decreased after three months treatment,but had no statistical significance.(4) HDP can improve nutritional status in maintenance hemodialysis patients.
