The Correlative Analysis Of Serum Heparanase And Vascular Disease And Pattern Of Syndrome In Type 2 Diabetes Mellitus

Objective:To investigate the correlation of serum heparanase(HPSE),vascular disease,risk factors and syndrome of TCM in type 2 diabetes mellitus and study potential role of HPSE to vascular disease and syndrome classification.Methods:120 patients with type 2 diabetes mellitus including 46 cases of extreme heat with yin asthenia syndrome and 74 cases of deficiency of both vital energy and yin syndrome were recruited in this research.The general clinical data as sex,age,duration and so on was collected.The clinical indexes to evaluate vascular disease including urine microalbumin-to-creatinine ratio(mALB/Cr),carotid intima-medial thickness(IMT) were measured.The risk factor indexes as fasting blood glucose(FBG),postprandial blood glucose(PBG),glycosylated hemoglobin A_1C(HbA_1C) and so on were assayed.All clinical data was analyzed by statistical way.Results:(1) The serum HPSE of group with FBG≥5.6 mmol/L was significantly higher than the group＜5.6 mmol/L(1.52±1.41 vs 0.92±1.25,P＜0.05).(2) The serum HPSE of patients older than 60 years with IMT≥0.75mm was significantly higher than IMT＜0.75mm(1.99±1.10 vs 1.22±1.36,P＜0.05),but there was no significant correlation between serum HPSE and urine mALB/Cr(r=-0.06282,P＞0.05),the differences of HPSE in teams divided by different levels of mALB/Cr were not statistical significant(P＞0.05).(3) The serum HPSE with syndrome of extreme heat with yin asthenia was significantly higher than deficiency of both qi and yin(1.65±1.32 vs 1.17±1.37,P＜0.05).Conclusion:(1) High level of FBG is accompanied with high serum HPSE. Hyperglycemia may be an important risk factor to serum HPSE of type 2 diabetic patients.(2) High level of IMT in early stage is accompanied with high serum HPSE. Serum HPSE of type 2 diabetic patients may reflect early change of macrovascular disease.(3) The serum HPSE of extreme heat with yin asthenia syndrome is significantly higher than deficiency of both qi and yin.Serum HPSE is closely correlated with the syndrome of extreme heat and be helpful to differentiate the two syndromes in type 2 diabetes mellitus.
