Objective : To evaluate high strength magnetic resonance cholangiopancreatography(MRCP) for syndrome differentiation of TCM in patients with obstructive jaundice, by analyzing the correlationship between the signs of MRCP and the syndrome differentiation of TCM. Methods: Routine MRI and MRCP examination on 66 patients with obstructive jaundice were performed with 1.5T Simens super-conductive magnetic resonance machine. Each case had been classified in either syndrome differentiation of Yin-Huang or syndrome differentiation of Yang-Huang by two professional doctors. Then the correlationship between characteristics of MRCP and the syndrome differentiations was analyzed. Final diagnosis were confirmed by surgical or ERCP pathology and clinic observation. Results: Of the 66 patients with obstructive jaundice,30 cases showed Yang-Huang, the main obstructive reasons of which included bile duct stones, inflammatory stenosis and congenital cyst; Of the 36 cases of Yin-Huang., the main obstructive reasons were cholangiocarcinoma, ampullar tumor and pancreatic head carcinoma. Yang-Huang was mainly caused by benign obstruction(90.0%), on the contrast , Yin-Huang was mainly caused by malignant obstruction(69.4%). The dilatation degree of bile duct in 36 Yin-Huang included 4 cases of light dilatation, 11 cases of moderate dilatation and 21cases of severe dilatation. The dilatation degree of bile duct in 30 Yang-Huang included 14 cases of light dilatation,12cases of moderate dilatation and 4 cases of severe dilatation. Majority of Ying-Huang presented vine-sign(75.0%)well dead-wood sign was more frequently found in cases of Yang-Huang(73.3 %). Conclusion: There had close correlationship between signs of MRCP and the syndrome differentiation of TCM. High strength MRCP was helpful in the syndrome differentiation of TCM.
