Objective: To distinguish the route of the branches of the pelvic artery and recognize angiographic manifestations for these branches, to present basic applied anatomy to be useful in pelvic diseases diagnose and for interventional catheterization and improving it s success rate.Materials and Methods: The branches of pelvic arteriography were studied by means of Abdominal 3D-DSA (in level of lumber 4) was taken in 69 adults (including42 females and 27 males), including the origin, length, diameter and bifurcation angle, all of them were compared in male and female groups. Three projected positions (contra lateral oblique 15~25°,25~35°and 35~45°view) were taken and the view aspects of uterine arteries were observed,the anatomic types of internal iliac arteries were observed.Results: (1) The origin of bilateral common iliac artery given off from the aorta between the inferior margin of the L4 and the superior margin of the L5, .the altitude of bilateral common iliac artery in males was higher than that of females. The angle between left and right common iliac artery in females and males were (62.4±7.99)°and (56.9±8.57)°, the angle of bilateral common iliac arteries in females was larger than that of males', there was significant difference between male and female groups (p<0.05).. The length of left and right common iliac artery in males were﹙61.8±4.14﹚㎜ and﹙64.6±4.04﹚㎜ , the length of left and right common iliac artery in females were﹙44.3±3.09﹚㎜ and﹙48.45±3.53﹚㎜, The length of right common iliac artery was longer than that of left common iliac artery. The bifurcation angle between internal and eternal iliac arteries were(24.4±5.61)°in left and (26.0±5.72)°in right, the angle in right was larger than that's of left, the bifurcation angle in elder men between internal and eternal iliac arteries were(27.4±1.19)°in left and(27.1±4.96)°in right, the bifurcation angle in younger men between internal and eternal iliac arteries were(23.1±4.80)°in left and(25.1±6.20)°in right, the angle of elder man was larger than that of younger man(P<0.05). The rates of iliolumbar artery'origin from superior gluteal artery and main truck of internal iliac artery were69.6% and30.4%, the rates of origin of lateral sacral arteries from superior gluteal artery and main truck of internal iliac artery were 62.3% and 37.7%. The rates of origin of obturator artery from superior gluteal artery, inferior gluteal artery and main truck of internal iliac artery were 66.7%, 30.4% and 2.9%. the rates of origin of superior vesical artery from front brunch and main truck of internal iliac artery were 85.0% and 15.0%.The rates of origin of uterine arteries from superior vesical artery, main truck of internal iliac artery ,inferior gluteal internal pudendal artery truck and superior gluteal artery were 69.0%,19.1%,9.5% and 2.4%. The uterine arteries and internal pudendal artery in younger females were wider than those of elder females, there was significant difference between male and female groups(P<0.05). (2) Internal iliac arteries can be divided into five anatomic types according to superior gluteal artery, inferior gluteal artery, and pudendal artery. (3) 25-35 degrees of contra lateral oblique view was the best projected position to show the opening and route of the uterine artery(P<0.05).Conclusion: (1) Because of the female patient s'larger angle between bilateral common iliac arteries , it is recommended that direct catheterization be used with percutaneous femoral artery puncture technique for contra lateral iliac procedures ,while in male patients , loop catheterization be much better.(2) 25-35 degrees of contra lateral oblique position is helpful for super selective catheterization of uterine artery.(3) Selective internal iliac arteriography in pelvic diseases could not only define the nature and location of tumor, but also estimate prognosis of the patients. According to internal iliac arteriography, the characteristic interventional procedures would better be carried out.
