| Objective: To investigate the prevention and treatment of hypo- calcemia after thyroid surgery .Methods: The change of serum calcium level of pre- and postoperative was analyzed in 49 patients with hypocalcemia after thyroid surgery . Hypocalcemia was defined as serum calcemia<2.20mmol/L . And serum PTH level was tested in 16 patients . The serum calcium level of postoperative changed with PTH level by statistical methods .Results: Hypocalcemia occurred in 49 cases (22.69%) after thyroid surgery . Of them , 5 cases were in total thyroidectomy , 36 cases in unilateral lobectomy with contralateral partial thyroid lobectomy , 7 cases in bilateral partial thyroid lobectomy , and 1 case in unilateral partial thyroid lobectomy . Transient hypocalcemia was identified in all patients , including 27 asymptomatic and 22 symptomatic which were serum calcemia <1.83mmol/L. There was a statistical significant different between pre- and postoperative serum calcemia in patients with total thyroidectomy, unilateral lobectomy with contralateral partial thyroid lobectomy and bilateral partial thyroid lobectomy(p< 0.05). The serum calcemia was the lowest point on second postoperative day. Serum PTH level was declined after operation but in normal range in 16 patients . Serum calcemia of postoperative reduced have relation with PTH declining.Conclusion:â‘ Hypocalcemia often occurs in patients after thyroid surgery.â‘¡The function of parathyroid may be changed with thyroid surgery.â‘¢The lower PTH after thyroid surgery may be one cause of hypocalcemia after thyroid surgery.â‘£Most patients were transient hypocalcemia and some of them occurred clinical symptoms. |
