| With the research of adipse tissue, we found that adipode tissue is not only a passive energy conservator ,but also a enormous endocrine system. It can secrete many kinds of adipose cell factor, including leptin,TNF-α,PAI-1,interleukin -6,resistin,IL-18,RBP4 and adipode cell factors that were newly found : adiponectin and visfatin. Adiponectin and visfatin is cell factors which were newly found and secreted by adipse cell , both of them all take part in the metabolic adjust of suger and fat , and there is a close relationship with obesity,insulin resistance and 2-diabetes, in the process of fetus growth and development , they probably play a important role. Objective :To measure Adiponectin and Visfatin level in maternal serum and neonatal serum and explore the correlation between adipocyte factors(Adiponectin and Visfatin )and fetus intrauterine growth.Methods : The levels of Adiponectin and Visfatin in serum were measured by enzymimmunoassay in 42 mothers and their 42 singleton term fetuses, anmong of which, 14 neonates were intrauterine growth restriction, 14 neonates were high birth weigh and 14 were appropriate for gestational age.Results :(1) Maternal serum levels of Visfatin in IUGR group(41.39±5.51 ng/ml)were significantly higher than that in control group(34.68±4.88 ng/ml)and that in high birth weigh group(37.34±4.40 ng/ml)(p<0.01, p<0.05); Maternal serum levels of Adiponectin in high birth weigh group(4.09±1.29 ug/ml) were significantly lower than that in control group(6.61±1.47 ug/ml) and that in IUGR group(6.39±1.30 ug/ml)(p<0.01, p<0.01).(2) neonatal serum levels of Visfatinin IUGR group (58.08±7.59 ng/ml)were significantly higher than that in control group (42.59±7.83 ng/ml) and that in high birth weigh group(48.48±9.06 ng/ml)(p<0.01, p<0.05); neonatal serum levels of Adiponectin in high birth weigh group(4.09±1.29 ug/ml) were significantly lower that in control group(7.68±1.51 ug/ml) and that in IUGR group(7.70±1.00 ug/ml) (p<0.05, p<0.05).(3) The Maternal serum levels of Visfatin had a significant positive relationship with that of neonatal serum levels(r= 0.720,p<0.001); the neonatal serum levels of Adiponectin were significantly higher than that in maternal serum level, there were no relationship between them ( r=0.661,p﹥0.05); .Maternal serum levels of Adiponectin were negatively related with their BMI( r=-0.318,p<0.05). Conclusion: The Maternal serum levels of Visfatin is slightly lower than that of neonatal serum levels and there are a significantly positive relationship between them , it revealed : besides maternal blood and placenta tissues ,visfatin originated from fetus , and adjust it's energy metabolism and growth by means of this way .The materal serum level of adiponectin is significantly lower than that of neonatal serum level and there were no relationship between them ,which revealed that adiponectin level in cord blood originated from fetus ,not placenta and maternal tissue . Fetus specificly secret cytokine of itself by adipose tissue , consequently adjust it's energy metabolism and growth.The levels of Visfatin in maternal serum and neonatal serum were significantly elevated in pregnancies with IUGR , so we presume :the pathologic conditions in pregnancy that lead to intrauterine growth restriction could be responsible for elevated maternal visfatin levels. Higher visfatin levels in pregnancy with intrauterine growth restriction may serve as an early marker with prognostic value for later development of insulin resistance or type 2 diabetes;wheras, the levels of Adiponectin were lower in pregnancies with high birth weigh. in woman of pregnancies with high birth weight ,because the pathologic conditions leads to lower adiponectin level , and sugar and fat metabolism is postponed , meantime energy metabolism disequilibrium, cause high birth weight... |
