Curative Effect Observation Of 102 Cases Of Neonatal Hypoxic-ischemic Encephalopathy

Objective:Neonatal hypoxic-ischemic encephalopathy (HIE) is a common central nervous system diseases of neonate,it is due to asphyxia in the perinatal period, which can lead to hypoxic-ischemic brain injury.The survival may have varing degrees of sequelaes of the nervous system,such as cerebral palsy,mental retardation,epilepsy,and abnormal behavior,which make a heavy burden on family and society.In our country, there are about 18 millions-21 millions new borns each year, so there maybe a large number of children with HIE.How to treat HIE for the improvement of children's neurological development, which is a topic of neonatal doctors at present.To investigate the clinical features and prognosis of the neonatal hypoxic- ischemic encephalopathy, and achieve one effective treatment.Methods:In this retrospective study, from February 2004 to May 2007, there were 102 HIE neonates who were treated in pediatric department of the second affiliated Hospital of Dalian Medical University. We analysis the clinical data and assistant examination of these cases.For all cases,brain CT scan was carried out at in-hospital and subsequent visit. For 28 cases of all, the neuropsychological development assessment was carried out.The gestational age,birth weight,Apgar score,Severity,CT result and prognosis of cases were recorded, these data were analysed by the statistical packages SPSS10.0.There were a significant when P<0.05.Results:1.The degree of clinical feature is basically same as the degree of CT results,most of cases were severe when the results of CT scan were also severe.2.The prognosis of the slight cases and slight CT change had a good prognosis,but the severe cases and severe CT change had a bad prognosis, the prognosis had a positive correlation with the clinical degree and CT degre.3. The neuropsychological development of 28 cases were nearly normal,while the gross and fine motor were a little bad.1 case was good, 7 cases were upper middle,14 cases were middle,3 cases were middle and lower levels, 3 cases were poor, same as clinical and CT degree.Conclusion:From this study about the comprehensive therapy for 102 cases neonatal hypoxic-ischemic encephalopathy,we got some conclusions as following:1. The prognosis of the mild neonatal hypoxic-ischemic encephalo- pathy was good, but part of the moderate and severe cases was not good.2. The CT results may predict the prognosis of the HIE cases.3.Early treatment could reduce the morbidity of bad sequel.
