Study On Clinical Characteristics, Growth And Development, Intelligence Of Child Patients With Human Immunodeficiency Virus Type 1

ObjectTo analysis the clinical feature,detect the development and the intelligence in 23 HIV-infected children,gain a systematic understanding of the impacts of HIV on the development and the intelligence of children,then to discuss the effect of highly active antiretroviral therapy (HAART) on the development and intelligence of HIV-infected children.Method1.23 HIV-infected children and 23 normal control children of 1.5～6 years.There is no difference in age,sex,the composition ratio.The physical examination,development and the intelligence are also tested.Besides,liver function and kindney function,blood cell count,CD4~+ cell count,virus load are detected in the HIV-infected group;some HIV-infected children are retested after HAART.2.To record the score of the weight,height,head circumference and intelligence,the clinical feature,calculate development quotient(DQ).To compare the difference in two groups and assess the effect of the HAART.ResultsThere are marasmus 20 cases(86.9%),diarrhea 14 cases(60.9%), fever 14 cases(60.9%),severe bacteria pneumonia 12 cases(52.2%), reccurrent upperresipiratory tract infection 10 cases(43.5%) and oral candidiasis 10 cases(43.5%) in 23 HIV-infected children.In the HIV-infected group and the control group,the average height respectively is(89.6±10.2) cm and(99.2±10.5) cm(P=0.03),the average weight is(12.7±2.7) kg and(16.5±3.2) kg(P＜0.01),the average head circumference is(49.0±1.8)cm and(49.8±1.5)cm(P=0.05 );There are significant differences in height and weight.There is no significant difference in head circumference(P=0.07)The scores of intelligence in two groups are as follows:Gross motor: 34.4±17.2 and 45.8±18.1(P=0.03),fine movement 33.6±15.4 and 43.7±21.3(P=0.07),adaptive capacity 31.3±13.2 and 40.6±18.3(P=0.07), language 31.9±13.1 and 43.9±17.5(P=0.05),sociality 32.9±12.3 and 44.0±18.9(P=0.02),DQ 73.4±9.5 and 97.3±6.8(P＜0.01).There is significant difference in DQ. There are increase trendency in height,weight and intelligence after 3-months and 12-months HAART,the average CD4~+ cells increase slightly;viral load is lower than the lower value of the test.Conclusions1.It is the first time to carry out the ivestigation of HIV-infected children in Hunan province.The infection mode is mother to child transmission in all infected children,the major symptom and complications are marasmus,fever,diarrhea;severe bacteria pneumonia,reccurrent upper resipiratory infection and oral candidiasis.2.The development index in HIV-infected children,such as the height and weight is lower than that of the same age of normal children.3.The score of intelligence of HIV-infected children is lower than that of normal ones,it is the first report in china up to now.4.We found that HAART is benefit to improve the development and growth,increase the intelligence,decrease the viral load and raise the CD4~+,it is also the first time to report in Chineses HIV-children.
