Objective To study The effect of pelvic autonomic nerve preservation and total mesorectal excision in male rectal carcinoma patients for presersing anus.Methods From April, 1999 to December, 2007, 528 male patients under 65 who were diagnosed with rectal carcinoma were admitted to our department, they received the operation of presersing anus after identified. From April, 1999 to July, 2000, we operated on 79 patients with the traditional method; we also operated on 127 patients with the total mesorectal excision from Auguet, 2000 to December, 2001; yet we operated on 322 patients with the pelvic autonomic nerve preservation and total mesorectal excision from December, 2001 to December, 2007oThese three groups were classified to control groups, TME groups and PANP+TME groups . We have a retrospective study of the distance from tumor's edge to anus, 3 year's survival rate, local recurence rate, urinary function and sexual function on these groups to search the difference among them..Results The distance from tumor's edge to anus in TME groups and PANP+TME groups was shorter than that in control groups. The three years survival rate of them have no significant difference( P>0.05). The local relapse rate of TME groups and PANP+TME groups was lower than that in control groups(P<0.05 ). The rate of urinary dysfunction and sexual dysfunction was gradualy reduced from control groups, TME groups to PANP+TME groups. Those three groups had signifcant difference.Couclusions TME technique could signifcantly reduce the local recurence rate, urinary dysfunction and sexual dysfunction. PANP combined with TME could reduce urinary dysfunction and sexual dysfunction much more.
