[Objective]The purpose of this study was to compare the CAG and IVUS features of atherosclerotic plaques with angina pectoris in different age groups.To elucidate the relationship between the luman remodeling of coronary artery lesion and the plaque features.[Methods]60 CHD patients with angina pectoris in the First Affiliated Hospital of Kunming Medical College from March 2009 to March 2010 were enrolled.They were divided into two groups by age as:age<60 years(n=30)in non-old group,24 male,6 female,SAP 10 cases,UAP 20 cases; age)60 years(n=30) in old group,male 16,female 14,SAP 18 cases,UAP 12 cases.All cases were examined by CAG and IVUS to analyze the criminal lesions.The detailed parameters involved external elastic membrane area(EEMA):luminal area(LA):plaque area(PA);area stenosis percentage; lipid-pool area;the ratio of lipid-pool area to plaque area;the thickness of fabric-cap;calcification incidence,calcification extent and calcification distribution.[Results]1) More single-vessel lesion was found in the non-old group by CAG than the old group(17cases,56.6%versus 9cases,30.0%,P<0.05),while more multiple vessel disease was found in the old group than the no-old group(12cases,40.0%versus 5cases,16.7%,P<0.05).2) The thickness of fabric-cap(0.51±0.33)mm in non-old group were smaller than the old group(1.11±0.75)mm,the lipid-pool area(3.7±2.1)mm~2 in non-old group were thicker than the old group(1.4±0.81)mm~2,P<0.05 for them.More unstable plaque in non-old group were found than the old group(16cases,53.3%versus 7cases,23.3%,P<0.05).3) More negative remodeling in old group were found than then on-old group(17 cases,56.7%vs 9 cases,30%,P<0.05).The calcified plaque (13cases,43.4%versus 5 cases 16.7%) and calcium arcⅢ(Scases,18.3% versus 0 case,0%) in old group were more than the non-old group(P<0.001).[Conclusion]1.More single-vessel lesion were found in no-old group,but more multiple vessel disease was found in the old group.2.More unstable plaque(small fabric-cap,thick lipid-pool)were observed in the no-old group.More stable plaque(thick fabric-cap,small lipid-pool)was observed in the old group.3.The old group has more negative remodeling,calcified plqaue and calcium arc than the non-old group.
