Objective:Oral Lichen Planus (OLP) is a common and frequently-occurring disease of oral mucosa, is an unknown aetiology inflammatory disease of non-infectious. It can occur in any part of the oral mucosa, most found in buccal mucosa, also involving the tongue and gums and other parts. According to the survey, the prevalence of the disease is 0.5-2.2%. There are variety of clinical manifestations of OLP, with symmetrical white reticulate or white plaque in the main, can be divided into texture type, papular type, plaque type, blister type, atrophy type and dissipated type. Now this lesion is recognized as a T-cells-mediated immunoreaction with a potentially premalignant property. Although its malignant transformation has not been fully substantiated, but it is considered as a precancerous condition. Histopathologically,it showed there may be disorders in epithelial cell proliferation and apoptosis, is also characterized by subepithelial band-like inflammatory infiltrate, variable numbers of intraepithelial mononuclear cells focused to the basal keratinocytes and degeneration/destruction of basal cells, as well as acanthosis, basal cell layer and lamina propria colloid body formation. The disease has a characteristics of protracted repeated, lengthy duration of the disease and long treatment. There are many ways of treatments of OLP,of which first-line treatment is topical application of glucocorticoid and it was also reported that cyclosporine treatment of OLP had a positive effect. But there is no way to completely cure for oral lichen planus.Previous reports revealed that keratinocyte growth factor (KGF) might promote the epithelial proliferation specifically with no effect on fibroblasts or endotheliocyte, thus could be used to treat ulceration caused by epithelial damage. In this study, we use MTT, PCR technology, and enzyme-linked immunosorbent assay technique to detect the effects of interleukin-1β(IL-1β) and dexamethasone (DEX) on the proliferation activity of vitro cultured normal human oral mucosa and oral lichen planus mucosa fibroblasts; As well as on fibroblasts in secreting and expressing keratinocyte growth factor in protein and gene levels. Providing a theoretical basis for the dose control of the clinical application of hormone treatment, at the same time provided new ideas for the clinical treatment of the disease.Methods:1. OLP (n=14) and normal control (n=11) tissues were defined clinically and pathologically. Culture normal oral mucosa and lichen planus mucous fibroblast cells in vitro. Selected third to seventh-generation cells were added to three kinds of concentration gradient of dexamethasone and interleukin-1βculture medium, continue to culture 48 hours.(1) Using MTT assay to detect the effect of two kinds different concentrations drugs on cell proliferation activity;(2) Using ELISA technique to detect the two drugs on fibroblast secretion of keratinocyte growth factor(KGF) protein in each group;(3) Using PCR technique to detect the effects of two drugs on each group fibroblasts expressing keratinocyte growth factor mRNA. The results of every group are expressed with the average optical density.2. Data of MTT and ELISA were analyzed by ANOVA and Student-t test,and expressed as the means±SD. Values of P<0.05 were considered statistically significant.Results:1. ELISA detection, keratinocyte growth factor secreted by the mucous of oral lichen planus is less than secreted by the normal oral mucosa fibroblasts. Between the OD values of each group there were significant differences (P<0.05);However, there were no significant group differences in the expression of KGF mRNA.2. Interleukin-1βcan enhance fibroblast proliferation activity; can promote the secretion of KGF protein and the KGF mRNA expression levels of fibroblast. The higher the concentration of IL-1βthe more obvious effect is.3. Dexamethasone can inhibit fibroblast proliferation activity; decline the secretion of KGF protein and the KGF mRNA expression levels of fibroblasts. The concentration of Dexamethasone increased the inhibition effect strengthened.Conclusion:The study found that Interleukin-1βcould enhance both the KGF protein secreting and the KGF mRNA expression of the fibroblast, and was positively correlated with IL-1βconcentration; however, Dexamethasone could inhibit both the KGF protein secreting and the KGF mRNA expression of the fibroblast, the extent of inhibition was enhanced with the increase of the concentration. Based on the role of keratinocyte growth factor, as well as the results of this experiment, provided a theoretical basis for the adaptation levy, medication time and dose control of the clinical application of hormone treatment, at the same time provided new ideas for the clinical treatment of the disease.
