Objective:Determination insulin like growth factor-1 (IGF-1), insulin like growth factor binding protein-3 (IGFBP-3), adiponectin, leptin in fetal cord blood of normal weight and fetal macrosomia. discuss role of insulin like growth factor-1 (IGF-1), insulin-like growth factor binding protein-3 (IGFBP-3), adiponectin and leptin in fetal growth and development, to provide evidence for the study of the pathogenesis of fetal macrosomia and its clinical prevention.Materials and Methods:1.1 Source Select 126 cases of infants born in Shandong Provincial Hospital, obstetrics from2009.06 to 2009.09. gestational age 36-42 weeks; male 64, female 62 cases. Depending on birth weight children were divided into the children of normal weight group (63 cases) and large child group (39 cases). Case selection criteria:(1) no fetal distress, congenital malformations and metabolic disease history, Apgar score9~10points;(2)maternal health, were singlet on pregnancies, no pregnancy-induced hypmertension, diabetes, thyroid disease endocrine diseases.1.2 Methods1.2.1 Sample collection and handling after delivery of the baby,6mL umbilical cord blood were collected before placental expulsion, separation of serum (3000r/min centrifugation for 10 minutes) after solidification, samples should be clear and transparent, to avoid hemolysis, placed in-20℃refrigerator Save, to be focused on determination of IGF-1, IGFBPP-3, leptin, adiponectin.1.2.2 Determination of Cord blood IGF-1, IGFBP-3, leptin and adiponectin Cord blood IGF-1, IGFBP-3, leptin and adiponectin were detected by enzyme-linked immunosorbent assay1.2.3 Determination of growth parameters weight, length, head circumference, foot length, placental weight were measured after neonatal umbilical off,and accuracy of 0.1kg and 0.1cm. Body mass index (BMI)=body weight (kg)/height (m)Results:1. Cord blood IGF-1, IGFBP-3, leptin and adiponectin levels were not significant gender differences.2. IGF-1 levels of Cord blood in macrosomia group was 70.34±1.44μg/L, normal weight children group was 43.43±0.68μg/L, the difference was statistically significant (t=75.525, P<0.001); umbilical serum IGF-1 levels and newborn weight, gestational age, head circumference, body length, foot length and placental weight were significantly positively correlated (P<0.01). IGFBP-3 levels of Cord blood in macrosomia group was 697.25±84.28μg/L, normal weight children group was 675.13±68.30μg/L, the difference was not statistically significant (t=0.912, P= 0.368).3. leptin levels of Cord blood in macrosomia group was 12.00±0.66μg/L, normal weight children group was7.11±0.53μg/L,the difference was statistically significant (t=25.667, P< 0.001); umbilical serum leptin levels and newborn weight, gestational age, head circumference, body length, foot length and placental weight were significantly positively correlated (P<0.01).4. adiponectin levels of Cord blood in macrosomia group was 92.50±10.52μg/L, normal weight children group was83.60±8.91μg/L,the difference was statistically significant(t=2.887, P=0.006); umbilical serum adiponectin levels and newborn weight, gestational age, head circumference, body length, foot length and placental weight were significantly positively correlated (P<0.01).5. Cord blood IGF-1 and adiponectin, leptin levels were significantly correlated (P <0.01), cord blood IGFBP-3 and adiponectin and leptin levels were positively correlated (P<0.05), but not significant.6. After statistical analysis only cord blood IGF-1 into the Logistic equationConclusion:1. IGF-1 levels of Cord blood in macrosomia group was significantly higher than normal weight children, and with the newborn weight, gestational age, head circumference, body length, foot length and placental weight were significantly positively correlated, suggesting that IGF-1 in fetal growth and development play an important role, may be the occurrence of great children.2. leptin, adiponectin levels of Cord blood in macrosomia group were significantly higher than children of normal weight group, and with the newborn weight, gestational age, head circumference, body length, foot length and placental weight were significantly positively correlated Tip of leptin, adiponectin in fetal growth and development play an important role, may be the occurrence of great children.3. Newborns cord blood IGF-1 and adiponectin and leptin levels were positively related, and only IGF-1 into the Logistic equation, suggesting that IGF-1 and fetal growth and development is directly related to leptin, adiponectin may by regulating the role of IGF-1 involved in fetal growth and development.
