Bioprosthesis Value Replacement Under Beating Heart In 87 Cases

Object:To evaluate effect of bioprosthesis valve replacement on beating heart with mild hypothermic cardiopulmonary bypass (CPB).Methods:A total of 87 patients of bioprosthesis valve replacement in beating heart with mild hypothermic extracorporeal circulation (30～32℃)between January 2003 and June 2009.There were 2 patients in NYHA class I,35 in classⅡtoⅢ,and 50 in class IV.56 with atrial fibrillation,11 with diabetes,23 with infective endocarditis.Among 87cases,20 had Mitral valve replacement,17 had Aortic valve replacement.20 had Mitral and Aortic valve replacement,15 had Mitrial valve replacement and Tricuspid valvuloplasty.15 were re-replacement.all patients contects by means of telephone.Results:The procedures underwent fluently and only one died early postoperatively,the total in-hospital mortality is 1.2%(1/87).length of stay was 13.4±2d (6-29d),only 67 cases in follow-up. The median follow-up duration was 3.4 years. Twenty patients lost follow-up and the rate was 77.1%.。Sixty patients returned to NYHA classⅠandⅡ,Twenty-six to classⅢ.one had cerebral embolism complications (1.2%) after six months, one died of infective endocarditis after one year.no re-replacement.Conculssions:The effect of bioprosthesis valve replacement on beating heart with mild hypothermic cardiopulmonary bypass is safe and excellent outcome, expecially for serious valvue diease.There were a few of related-anticoagulant complication of postoperation.
