Objective: In preoperative, intraoperative and postoperative, determine IgA, IgG, IgM, C3 and C4 in patients undergoing arrested or beating heart valve replacement, understanding changes information and discussing on variation regularity of the body's immune function,for future prevention and treatment of cardiopathic patients under cardiopulmonary bypass caused damage by changes in immune function are significant. Meanwhile, compare the IgA, IgG, IgM, C3 and C4 in patients undergoing arrested heart or beating heart valve replacement, explore immune function variation regularity of the two different surgical ways.Methods: Randomly selected 21 patients(control group) undergoing beating heart valve replacement and 14 patients(experimental group) undergoing arrested heart valve replacement in patients with rheumatic heart disease. Use immunoturbidimetry to determine IgA, IgG, IgM, C3 and C4 in patients of the two groups in preoperative, intraoperative and postoperative. Spss 13.0 statistical software was applied for data analysis, P-value less than 0.05 were considered significant. Used the t-test and analysis of variance compared the data of two groups.Results: 1.The operating and CPB duration were shortened,compared with the control group.2. IgA(g/l): It was recovery at postoperative 24h in the experimental group, itwas postoperative 3d in the control group. The experimental group of it wassignificantly higher than the control group at postoperative 24 h.3.IgG(g/l): It was recovery at postoperative 7d in the experimental group, butuntil this time has not yet recovery in the control group. The experimental groupof it was significantly higher than the control group at after CPB 15min and30min, postoperative 24 h.4. IgM(g/l): The same to IgA.5. C3(g/l): It was recovery at postoperative 24h in the experimental group, but until this time has not yet recovery in the control group. There were even much higher than the preoperative of it in the experimental and control group after postoperation 3d and 7d. The experimental group of it was significantly higher than the control group at after CPB 30min and postoperative 24 h,but significantly lower than the control group at postoperative 7d.6. C4(g/l): It was recovery at postoperative 24h and significantly higher after postoperation 3d and 7d in the experimental group, but it was recovery until postoperative 3d and significantly higher after postoperation 7d in the control group. The experimental group of it was significantly higher than the control group at postoperative 24 h.Conclusion: The patients undergoing heart valve replacement under cardiopulmonary bypass caused the lower level of immune function in intraoperative and postoperative 24h-7d, or even longer. Compared to the arrested heart valve replacement, the patients' level of immune function undergoing beating heartvalve replacement were recovery faster. It's demonstrated that beating heart valve replacement superior to arrested heart valve replacement in the influence of immune function.
