A Study On Correlation Among The HBV-DNA Level, HBV Genotype (Subtype) And Familial Elustering Of Hepatocellular Carcinoma In The High Incidence Area Of Guangxi

Objective: To study the association among the HBV-DNA replication level, HBV genotype (subtype) and the other relevant risk factors and familial elustering of hepatocellular carcinoma in the high incidence area of Guangxi.Methods: Questionnaires for relevant risk factors of HCC and peripheral blood samples were collected from 209 members of 25 HCC high incidence families and 486 members of 66 control families, of Zhuang People, in Fusui county of Guangxi Province, with very high incidence rate of HCC. HBsAg in serum were tested with ELISA. Quantification of HBV-DNA replication level were determined by PCR-Fluorescence Probing. HBV genotype and its subtype were determined by multiple primers PCR and PCR-RFLP, respectively.Results:⑴Among the high risk family group and control group, the positive rate of HBV-DNA in serum were 31.1% (65/209) and 16.5% (80/486), the difference was significant (χ2=18.971, P=0.000), Odds Ratio (OR) was 2.291 (95%CI=1.569~3.345). The replication level of HBV-DNA in the high risk family group were higher than in the control group, the difference was significant (χ2=10.998, P=0.012).⑵The infection rate of HBV in the high risk family group, genotype B was 13.9% (29 cases), genotype C was 30.1% (63 cases), genotype B+C was 13.4% (28 cases). The infection rate of HBV in the control group, genotype B was 7.8% (38 cases), genotype C was 14.4% (70 cases), genotype B+C was 6.6% (32 cases). The infection rates of HBV among the high risk family group and control group, the difference were significant.⑶The infection rate of HBV in the high risk family group, gene subtype B2 was 13.9% (29 cases), gene subtype C1 was 14.4% (30 cases), gene subtype C2 was 12.9% (27 cases), the other subtype of genotype C was 2.9% (6 cases), the gene subtype B2+C1 was 6.7% (14 cases), and the gene subtype B2+C2 was 6.7% (14 cases). The infection rate of HBV in the control group, gene subtype B2 was 7.8% (38 cases), gene subtype C1 was 8.6% (43 cases), gene subtype C2 was 5.1% (25 cases), the other subtype of genotype C was 0.6% (3 cases), the gene subtype B2+C1 was 4.7% (23 cases), and the gene subtype B2+C2 was 1.9% (9 cases).⑷After analysis by the non-condition logistic regression, we found that HBV gene subtype B2 infection, HBV gene subtype C1 infection, HBV gene subtype C2 infection, HBV gene subtype B2+C1 combined infection, HBV- DNA high level in serum and corn intake are the major high risk factors of the familial susceptibility to HCC.Conclusion: HBV-DNA replication level, HBV genotype (subtype) in serum, and the other relevant risk factors of HCC may associate with the familial susceptibility of hepatocellular carcinoma in the high incidence area of Guangxi. In the high incidence area of Guangxi, genotype C of HBV is mainly, the next is genotype B, and the genotype B+C combined infection is frequent. The gene subtype of genotype B are mainly B2. The gene subtype of genotype C are mainly C1, the next is C2.
