The Clinical Analysis Of 90 Patients Suffered From Subarachnoid Hemorrhage With Negative Cerebral Angiography

Objective:To discuss the cause, clinical features and the prognosis of the negative results of initial cerebral digital subtraction angiography(DSA) with subarachnoid hemorrhage(SAH).Methods:To analyze retrospectively 261 cases in our department of neurosurgery which diagnosis was spontaneous subarachnoid hemorrhage through CT and/or lumbar puncture between June 2006 and December 2008. Make the contrast analyze of the 90 cases that have negative results of the DSA examination and 171 cases that have positive results of the DSA examination.Results:Among the patients who have negative results of the DSA examination,11 patients have hypertension,4 patients have Diabetes,17cases have smoking behavior. All patient's Hunt and Hess Index belong to levelâ… -III. After treatment, the average time of the intracranial hypertension is 3.5 days. And the average time of the disorders of consciousness is 0.2 day. The average score of NIHSS Index is 0.4. On CT imaging, the number of cases with tiny bleeding of brain is 62, and there are 34 cases conform to perimesencephalic nonaneurysmal subarachnoid hemorrhage(PNSH).Conclusion:â‘ Clinical symptoms is mild in the SAH's patients with negative cerebral angiography.â‘¡About 37.7% of the patients who have negative results of the DSA examination belong to a special style perimesencephalic nonaneurysmal subarachnoid hemorrhage(PNSH),the first reason of SAH's patients with negative cerebral angiography is mainly due to PNSAH.â‘¢In the treatment the SAH's patients with negative cerebral angiography need drug or symptomatic treatment only and therapeutic effect is better; The complication of SAH's patients with negative cerebral angiography is fewer, mortality is lower and prognosis is good.
