Clinical Study On Intervention Of Hypercoagulable State After Hip Fracture With Self-prepared Decoction Of Huoxue Tongmai

Purpose:To investigate further the epidemiological law of hypercoagulable state after hip fracture and the relationship between this state and deep vein thrombosis (DVT); to explore the influence of self-prepared decoction of Huoxue Tongmai on hypercoagulable state after hip fracture, for providing the preve-ntion and treatment of clinical DVT a scientific basis.Methods:From 12,2009 to 2,2010, all resident patients of traumatic hip fra-cture (including femoral neck fracture, intertrochanteric fracture, acetabu-lar fracture)in department ten of orthopedics in Foshan hospital of TCM were selected. Blood samples were drawn from all patients after their hospitaliza-tion to measure the following indices:D-Dimer, thrombelastograph(TEG), platel-et count(PLC), granulemembrane protein 140(GMP-140), TAT,Von willebrand fact-or (vWF).Aslo lower limb color Doppler sonography examination was done. Then 72 patients in hypercoagulable state were screened out and randomly and evenly divided into two groups. In these two groups the patients were given respe-ctively the self-prepared decoction of Huoxue Tongmai and Chinese medicine of promoting blood and removing stasis (pseudo-ginseng oral liquid, Yunnan Baiyao capsule). And blood samples were drawn from all patients on the day before operation, the 1st and 10th day after operation to measure the indice-s, and lower limb color Doppler sonography examination was done on the 10th day after operation.Results:(1)The hospitalization day indices of D-Dimer, G, TAT, vWF, GMP-140 of all patients were higher than normal, PLC, R, K, MA, and a angle were in the normal range. (2)The PLC on the day before operation were decreased compared with the hospitalization day, increased on the 1st day after operation compared with the day before operation, and continued until the 10th day after operation. The indices on the day before operation differ significantly between the two groups (P<0.05). (3)The D-Dimer on the day before operation were significantly decreased compared with the hospitalization day, increased on the 1st day after operation compared with the day before operation, and decreased to close to preoperative levels on the 10th day after operation. The indices on the 1st day,10th day after operation differ significantly between the two groups(P <0.05). (4) R and K on the day before operation were larger than those on hos-pitalization day, though a angle, MA and G smaller; on the 1st day after operation R and K were smaller than those before operation, though a angle, MA and G larger; and R and K were larger on the 10th day after operation than those on the 1st day after operation, although a angle, MA and G smaller. The indices of R and G on the day before operation differ significantly between the two groups(P<0.05). The indices of G on the 1st day after operation differ significantly between the two groups(P<0.05). The indices of each parameter on the 10th day after operation differ significantly between the two groups (p<0.05). (5) TAT on the day before operation were decreased compared with the hospitalization day, increased on the 1st day after operation compared with the day before operation, and decreased on the 10th day after operation compared with the the 1st day after operation,but was still higher than normal levels. The indices on the 1st day,10th day after operation differ significantly b-etween the two groups(P<0.05). (6) GMP-140 on the day before operation were decreased compared with the hospitalization day, increased on the 1st day after operation compared with the day before operation,and decreased on the 10th day after operation compared with the the 1st day after operation, but was still higher than normal levels. The indices on the day before operation,1st day,10th day after operation differ significantly between the two groups(P<0.05). (7) vWF on the day before operation were decreased compared with the hospitaliz-ation day, increased on the 1st day after operation compared with the day before operation, and decreased on the 10th day after operation compared with the the 1st day after operation, and group A decreased to normal levels, though group B were still higher than normal levels. The indices on the day before oper-ation,10th day after operation differ significantly between the two groups (P <0.05). (8)The incidence of DVT between the two groups had no significant difference (P>0.05).Conclusion:(1)The hypercoagulable state were observed in most patients after hip fracture,and most accompanied by several abnormal coagulation indexes. In a related factors, smoking, hypertension, diabetes mellitus might be more close. (2)Self-prepared decoction of Huoxue Tongmai could reduce D-Dimer, TAT, GMP-140, vWF, shorten a angle, MA, G of TEG, and extend R, K after hip fracture, suggesting that the prescription could significantly improve the hypercoagu-lable state post hip fracture;also it did not increase bleeding and other serious adverse events, suitable for early application of hip fracture pati-ents.
